The Montana Department of Public Health and Human Services (DPHHS) has a career opportunity within the Behavioral Health and Developmental Disabilities Division, Treatment Bureau. The Behavioral Health and Developmental Disabilities Division (BHDD) aims to implement and improve a statewide system of prevention, treatment, care and rehabilitation for Montanans with mental disorders or addictions to drugs or alcohol. By teaming up with behavioral health specialists throughout Montana, BHDD provides substance abuse and adult mental health services for those in need of support. The Care Transitions Section of BHDD will provide Critical Time Intervention (CTI) services to Montanans experiencing a major life transition, specifically low-income Montanans transitioning from institutional settings. The Care Transitions section will coordinate care by establishing community partnerships and facilitating access to services and supports individuals need during their transition, helping them move into the least restrictive and most suitable environment where they can thrive. This position provides clinical and programmatic oversight, coordination, training, and guidance to Care Transition Section Staff regarding the provision of CTI services, programs, policy and procedure. The position also integrates mental health professional expertise in the oversight of the program and literature review, data analysis, and quality assurance and improvement of the Care Transitions Section. The position supervises 7 positions. Other responsibilities include: 1) Creating and executing a plan to implement CTI services; 2) Providing training and mentoring to Care Transitions Coordinators; and 3) Developing, evaluating, and adjusting the CTI worker position and job duties to ensure they meet the objective of aiding individuals successfully transition back into their community after institutionalization.
