About The Position

The Career Services Case Manager will work to build and foster professional relationships with individuals served on her/his caseload. The Case Manager will work in conjunction with other departments, individuals, families, residences, funding sources, and job sites to ensure that services meet the individual needs and promote individual choices. The Case Manager will maintain confidential files for each individual and complete all necessary documentation to meet regulatory agency requirements.

Requirements

  • High School diploma required (BA degree in Human Services or related field preferred)
  • At least 1 year of experience supporting adults with developmental disabilities is preferred
  • A passion and dedication to supporting our people is a must
  • Valid Driver’s license, reliable transportation, and acceptable driving record
  • Must pass company background and reference checks

Responsibilities

  • Build and Foster professional relationships with individuals on her/his caseload, family members, and members of the ISP team to maximize an individual’s human dignity, community integration and Job opportunities.
  • Case Manager Float should routinely spend time with individuals on her/his caseload, in all settings across all Career Service services including jobsites and CBDS groups to support them in completing their personal and professional goals.
  • Maintain routine and effective communication with members of the ISP team regarding progress, service planning, and incident reporting within the boundaries of confidentiality.
  • Schedule meetings routinely to discuss individuals’ progress and/or needed support areas.
  • Monitor follow-through and effectiveness of support plans with department staff and Interdisciplinary team in cooperation with the department manager. Provide guidance to department staff in completing required documentation.
  • Ensure that all necessary documentation is completed in a timely manner. Ensure ISP quarterly and yearly progress reports are completed and uploaded onto the HCSIC website.
  • Route all required documentations to members of the ISP team such as ISP sign off sheets, incident reports, etc. File all required paperwork in the Individuals’ confidential file.
  • Actively provide input and coordinate with other departments and external referral sources to assure needed and desired services are being provided.
  • Plan for and participate in planning ISP meetings actively advocating for Individuals’ needs and contributing to overall goals of the department and individuals supported.
  • Participate and assist with intake process and orientation of new individuals to Career services.
  • Coordinate or complete assessments/evaluations to determine support levels and support planning. Conduct re-evaluations on an as-needed basis.
  • Provide direct support coverage and break coverage in the absence of skills instructors as needed. This includes jobsites and one-on-one job training.
  • Transport people supported in personal vehicle to and from work and/or community activities when no other agency vehicles or other options are available.
