Summary: Responsible for ensuring clients leave treatment with a solid aftercare plan, safe housing, and further needed treatment. Assist in meeting overall census and budget goals.
Requirements
- ADT certificate or active licensure to provide SUD services in Maryland
- Excellent oral and written communication and interpersonal skills.
- Sound clinical judgment and excellent clinical skills.
- Ability to problem-solve by gathering and analyzing information.
- Ability to handle a crisis situation and react appropriately.
- A working knowledge of federal and state standards as well as regulating body standards.
Responsibilities
- Provides quality direct and indirect client care to individuals with substance abuse or chemical dependency problems.
- Assesses clients' continuing care/referral needs.
- Establishes rapport with clients.
- Obtains the necessary information and releases from clients, family, referral sources, and other appropriate resources.
- Makes referrals to Mental Health and Drug & Alcohol facilities for continuing care.
- Sets up Halfway House interviews and placements.
- Works closely with counselors and members of the multi-disciplinary team.
- Attends and participates in program, clinical, and administrative meetings.
- Actively supports the facility’s ongoing referral program.
- Participates in mandatory orientation and training activities.
- Other duties as assigned.
