Case Management Assistant
About The Position
Clear the path for specialized care by securing necessary authorizations and managing clinical denials, ensuring patients have uninterrupted access to essential ancillary services. Advocate for patient choice by identifying and documenting ideal post-acute referrals, partnering with facilities and agencies that best meet the patient’s unique recovery goals. Expedite the journey home by coordinating the rapid transfer of clinical documentation to receiving providers, minimizing discharge delays and ensuring a safe transition of care. Bridge the communication gap by acting as the primary liaison between clinical teams, admissions, and insurance payers to keep all stakeholders aligned on the patient's status and discharge timeline.
Requirements
- High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
- Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
Nice To Haves
- Knowledge of medical terminology proficiency with Google suites
- Epic experience and remote work experience preferred.
