Under the supervision of the Manager of Case Management, the Case Management Discharge Assistant works with the Case Managers, Social Workers and Director of Case Management to coordinate department projects and data. The Case Management Discharge Assistant maintains appropriate documentation for management of the process. Department Specific Competencies Reviews patient list in order to appropriately refer patients to the correct agencies or offices for services daily. Ensures payer communication with reference #s and authorizations for benefits coverage for any outside services including MCARE. Delivers daily the QI0 2nd letter to MCARE patients 48 hours prior to DC for compliance with the CMS. Attends daily rounds whenever possible for information regarding DC plan for each patient. Composes list of resources for Case Managers and Social Workers as needed. Enters documentation integrity information in the computer and keeps a log. Coordinates department statistics and forwards information to budget department and director. Oversees Insurance authorization, reviews, and denials. Works with Revenue Cycle areas regarding Blue Cross billing process. Covers Department Secretary responsibilities on an as needed basis. Demonstrates role in department specific patient safety. Reports errors, knowing that Torrance Memorial engages in a positive environment for the correction of errors (Non-punitive). Participates in hospital performance improvement activities when assigned.
