About The Position

The Targeted Case Management (TCM) Manager oversees the daily operation, integrity, and performance of the outpatient targeted case managers. This position provides limited direct services, provides administrative and clinical supervision of TCM staff who provide Severe Emotional/Behavior Disorders (SED), Severe Mental Illness (SMI), and/or Substance Use Disorder (SUD) case management services. This manager leads community and interagency partnerships, monitors productivity and outcomes, and ensures compliance with all regulatory, ethical, and organizational standards. This role coordinates and assists in providing core TCM, Severe Emotional/Behavior Disorders (SED), Severe Mental Illness (SMI), Substance Use Disorder (SUD) and co-occurring medical training as required by Department of Behavioral Health, Developmental and Intellectual Disabilities (DBHDID).

Requirements

  • Minimum of a Bachelor’s degree in Psychology, Social Work, Marriage and Family Therapy, Professional Counseling or Human Services field that is billable per Department of Medicaid Services regulations.
  • At least three years of experience working with adults with Severe Mental Illness (SMI) diagnosis or youth with Severe Emotional/Behavior Disorders (SED) diagnosis.
  • Valid driver’s license, reliable transportation, and proof of automobile insurance.
  • Must pass Criminal Record Check and Sexual Offender Registry check.
  • Strong oral and written communication skills.
  • Proficiency with Microsoft Office and ability to learn EHR systems.
  • Ability to work flexible hours as required for supervision, crisis needs, or community engagement
  • Ability to sit for extended time for meetings, documentation, and clinic services.
  • Ability to stand, walk, bend, and move within clinic and community settings.
  • Must maintain confidentiality and adhere to all regulatory and ethical standards.

Nice To Haves

  • Working knowledge of Microsoft Office Products including Outlook, Word and Excel.

Responsibilities

  • Leadership & Operations Oversee daily operations of the TCMs, ensuring smooth workflow, service delivery, and adherence to agency policies.
  • Monitor revenues, expenses, wraparound fund utilization, service trends, productivity, caseload distribution, documentation timeliness, and clinical outcomes.
  • Ensure that the TCMs are compliant with federal, state, ethical, and organizational standards.
  • Recommend, implement, and refine systems or processes that improve efficiency, clinical quality, customer service, and revenue generation.
  • Work collaboratively with support staff and clinic coordinators to maintain professional, organized, and client-centered clinic environments.
  • Oversight & Staff Support Provide training, direct supervision, consultation, and performance feedback to all TCM staff assigned, on average 8-12 staff.
  • Assign cases, ensure proper coverage of cases when staff are absent, including the provision of direct service when needed.
  • Conduct performance evaluations annually or as needed.
  • Provide training, coaching, and professional development for TCM staff.
  • Provide minimum monthly group TCM/billing supervision and required individual supervision.
  • Ensure staff comply with agency policies, service requirements, evidenced based practices, and documentation standards.
  • Provide resource linkage and education to community agencies, law enforcement, schools, businesses, and other partners.
  • Support community engagement efforts such as outreach, marketing, and program visibility initiatives.
  • Program Coordination & Community Relations Participate in or assign staff to community relations activities, representing clinic programs to the public and community stakeholders.
  • Collaborate with directors, office managers, outpatient clinic managers and interdisciplinary leaders to support strategic goals.
  • Quality Assurance & Compliance Conduct regular documentation reviews, chart audits, and clinical quality checks.
  • Provide feedback, corrective action, and coaching to ensure compliance with agency, payer, and regulatory standards.
  • Work proactively to address consumer and TCM concerns.
  • Ensure the training curriculum for TCM, Severe Emotional/Behavior Disorders (SED), Severe Mental Illness (SMI) and Substance Use Disorder (SUD) meet the requirements of Department of Behavioral Health, Developmental and Intellectual Disabilities (DBHDID) for staff to provide necessary services.
  • Promote adherence to agency core values, ethical practice, and trauma-informed service delivery.
  • Other Duties Attend meetings, leadership trainings, conferences, and organizational activities as assigned.
  • Serve on committees or project teams as needed.
  • Perform additional duties assigned by the Director.
