Working with a supportive management staff and a company that puts their clients and their employees first, Crossroads Roads Health is an organization built on mutual respect and a work-life balance that is in need of a passionate, empathic individual to perform the following tasks: The Case Manager, Crisis Services offers case management services within the home and community setting. This position is responsible for primarily individual services. This provider will engage as a member of a collaborative team. With a Trauma Informed approach, this position will be a representative for the organization and how we offer integrated services for recovery and mental health. Positions may be specific to Youth Services, Adult Services, or a combination of both and may be specific to general population, intensive services, or a combination.

Requirements

  • High School Diploma or equivalent with 3 years of experience.
  • Associate’s Degree in Human Services/Social Work or related field with 1 year experience preferred.
  • QMHS/QBHS within 15 days of hire (employer provided)
  • Must have an acceptable driving record 4 (four) points or less, reliable transportation and carry automobile insurance with minimum liability of $100,000/$300,000, and property damage of $50,000.
  • Work in home and community within Lake County. providers to clients also travel to surrounding counties as needed. Must be comfortable making home visits throughout the community and transporting clients in own car.

Responsibilities

  • Provides therapeutic interventions in the home or community and linking clients to resources
  • Assesses clients’ development, psychological and psychiatric needs in order to make referrals.
  • Advocates on behalf of clients.
  • Monitors clinical symptoms.
  • Functions as a gatekeeper for other resources.
  • Performs crisis intervention, as needed.
  • Meet monthly productivity metrics
