About The Position
The Case Manager is a graduate level professional that has demonstrated expertise in providing social and emotional support to patients with chronic conditions. The position provides services to the Geriatric service line, working collaboratively with an interdisciplinary care team to optimize patient’s quality of life by addressing patient’s needs holistically.
Requirements
- Master’s degree in Social Work required.
- 2 years of case management, home healthcare experience required.
- Current knowledge of standards of care and clinical guidelines for patients with chronic illness
