This position provides direct case management to adults with serious mental illness and co-occurring conditions who are experiencing homelessness or housing instability. The Case Manager II works alongside clients to assess needs, develop individualized service plans, access resources, and strengthen skills for long-term stability. They serve as both advocate and connector - helping clients navigate complex systems of care while promoting dignity, safety, and independence. The role collaborates closely with internal teams and community partners to ensure clients receive consistent, trauma-informed support. This position requires a valid Minnesota driver’s license, a reliable vehicle, and proof of insurance.
