SUMMARY DESCRIPTION OF WORK This position connects those experiencing homelessness with housing in the form of temporary financial assistance and supportive services in Douglas, Sarpy, and Pottawattamie Counties. This Case Manager will work to assess, verify eligibility, assist clients with locating and securing housing, and provide case management/supportive service to clients in the program using the Housing First model. Intensive case management responsibilities begin at this level. Educational requirements are specific. Decision making, problem solving, and highly developed interpersonal skills are critical. This position requires high level of social work skills, practice, and knowledge.
