The Case Manager Liaison is a multi-faceted position that will give you variety in your day. This role will be responsible for assisting with office, caregiver, and client coordination as well as generating brand awareness in the community. We are looking for a highly professional, highly motivated, trustworthy, goal-oriented individual who loves the drive for success. The candidate should have a minimum of 1 year healthcare experience, RN, LPN, or CNA a plus, though not required. Exceptional written and verbal communications skills are a must as well as having the ability to interact effectively with professionals in a variety of settings including medical settings and the community. Must be tactful and determined to find and speak with key decision-makers. Excellent follow-up, planning and organizational skills are of utmost importance as well as having a positive and professional business image. Must have basic knowledge of Microsoft Office Suite as well as be able to learn and navigate other platforms with ease. Must be passionate about helping seniors/elderly and others live with dignity. Please note: This position has a base pay plus commission.

Requirements

  • College degree and health care, home care, RN, LPN, or CNA experience is a plus though not required.
  • A reliable means of transportation with current insurance and valid driver’s license.
  • Exceptional written and verbal communications skills are a must as well as having the ability to interact effectively with professionals in a variety of settings including medical settings and the community.
  • Must be tactful and determined to find and speak with key decision-makers.
  • Excellent follow-up, planning and organizational skills are of utmost importance as well as having a positive and professional business image.

Responsibilities

  • Provide case management and coordination of clients and caregivers.
  • Ensure excellent client centered care and communication.
  • Assist with coordination of referrals and in home assessments.
  • Complete detailed care plans.
  • Assist with recruitment efforts of caregivers including performing interviews and validating candidates.
  • Complete in-person candidate orientation and assist in building orientation program.
  • Build and maintain lasting relationships with new and existing referral sources and community organizations.
  • Conduct in-person calls, ensuring a face-to-face conversation with key decision makers along with dependable follow up.
  • Ensure detailed and diligent documentation.
  • Cover for staff on an as needed basis, including after-hours phone coverage.
  • Provide excellent communication in all aspects of position.
