About The Position

Lutheran Community Services NW is a non-profit organization dedicated to serving communities throughout Washington, Oregon, and Idaho with a wide range of services for individuals of all ages and backgrounds. All LCSNW team members will contribute to our mission of partnering with individuals, families and communities for health, justice and hope. Please note this position requires candidates to be speak, read, write in English, Dari and/or Pashto. The Case Manager assists and supports refugee, asylum, and immigrant clients to help them resettle and integrate into their new community and provide the resources and support to achieve self-sufficiency.

Requirements

  • Education and/or Experience: High school diploma or equivalent is required.
  • General Knowledge, Skills and Abilities: Ability to work well under pressure and work effectively in a multicultural setting, demonstrating excellent organizational skills and interpersonal skills. General experience with Google Suite and Microsoft Office365.
  • Transportation: This position requires valid state driver’s license,insurance and reliable transportation for business purposes, such as between work locations and community settings. This position is responsible for driving clients in a personal and/or agency rental.
  • Language Skills: Excellent communication skills with the ability to read, write, and speak in English, Dari or Pashto.
  • Clearance: Standard LCSNW criminal clearance, including national sex offender. Motor vehicle records, valid driver’s license and auto insurance are required.

Responsibilities

  • Maintain case management for refugee clients in accordance with state and federal-funded contracts
  • Work with clients to develop strengths-based service plans to identify barriers to self-sufficiency and outline the necessary steps to overcome these barriers
  • Connect clients with community networks and support systems to ensure their long-term integration into the community (this may include working closely with volunteers who are supporting clients)
  • Provide transportation for clients, including scheduling medical transportation or ensuring clients understand and have access to public transportation, to attend required appointments and ensure that language access is provided at these appointments
  • Provide in-home services to refugees including case management, needs assessments, cultural orientation, education, crisis support, information and referrals as needed.
  • Keep in regular contact with active cases per contractual requirements, including home-visits, office meetings, community orientation, and phone calls
  • Maintain organized case files, complete accurate progress notes for all services provided, submit necessary reports in a timely manner, and keep shared tracking forms up to date
  • Coordinate needed services through referrals to refugee service providers including the Health Department, Employment Services, Social Security, DSHS and ESL providers
  • Ensure that clients are being served in a linguistically and culturally appropriate manner and provide education and advocacy when necessary
  • Maintain compliance with all relevant program/department requirements, including and not limited to adhering to all record-keeping and reporting requirements.
  • Model and encourage direct, professional communication among staff members and facilitate development of a positive staff support network.
  • Demonstrate awareness and sensitivity in the delivery of client services as it relates to the cultural and socioeconomic characteristics of the populations served.
  • Other duties as assigned.
