About The Position

Direct Clinical Service - Provides mental health case/care management services to adults, children and families; provides collaborative care planning, referral and monitoring of treatment. Coordinates services needed within the agency and within the community.

Requirements

  • Working knowledge of community resources.
  • Excellent verbal and written communications skills.
  • Strong interpersonal and customer relations skills
  • Organizational, time management and problem-solving skills.
  • Basic computer knowledge and clinical recordkeeping experience
  • Knowledge regarding psychiatric rehab and understanding of recovery model.
  • Understanding of and competence in serving culturally diverse populations.
  • Ability to set limits, confront behaviors and redirect.
  • Ability to assess, teach and model skill development techniques
  • Ohio driver's license
  • Proof of automobile liability insurance (minimum $100,000)
  • High School diploma or equivalent required.
  • Experience in working in the mental health and/or substance abuse field.

Nice To Haves

  • Bachelor’s degree preferred in a mental health related field.
  • LSW, LPC licensure not required.

Responsibilities

  • Works collaboratively with Outpatient Clinician to connect clients with resources and schedules appointments as needed.
  • Communicates with the treatment team to improve quality of care to patient as well as quality outcomes.
  • May provide transport for clients to services.
  • Maintains productivity and documentation standards.
  • Commitment to the recovery model philosophy and standards, and foster an environment that supports recovery for persons served.
  • Assertive outreach to clients not engaging which may include telephone calls or home visits.
  • Performs crisis assessments on patients requiring emergent treatment, assessing level of risk and providing appropriate level of care recommendation.
  • Assist in completion of the NOMs and reporting necessary quality data to key stakeholders.
  • Performs related office/administrative duties, such as scheduling patients, answering phones, and assist in facilitating telehealth appointments.
  • Attends staff meetings, supervisions and trainings
  • Promotes a client/driven, strength-oriented service.
  • Adheres to the Netcare's mission statement and policy and procedures.
  • Follows guidelines to ensure quality assurance.
  • Performs other duties as assigned.
