About The Position

Project Longevity is a comprehensive initiative to reduce gun violence in Connecticut’s major cities. Through Project Longevity, community members and law enforcement directly engage with members of groups that are prone to commit violence and deliver a community message against violence, a law enforcement message about the consequences of further violence and an offer of help for those who want it. Our Project Longevity Care Coordination Program is a resource for Project Longevity team and law enforcement to support these group members, families and other’s impacted by violence. The Case Manager position is a direct care position working with individuals and families referred by the Project Longevity Program to provide wrap-around and trauma informed service coordination. The Case Manager is a member of the Service Coordination team that seeks to ensure participants receive tailored services to meet all of their unique needs. The role includes all facets of case management from initial assessment, identification of participant needs, case planning, prevention and reintegration interventions, and services, as well as facilitating links with the participant's natural support network and external providers. Core knowledge that is fundamental to the Case Manager includes: Knowledge of social services, neighborhoods, political landscape and community stakeholders throughout greater New Haven Exceptional ability to work in a collaborative environment within multiple organizations Experience working with individuals and families in need of unique and specialized services including but not limited to physical/medical health, behavioral/mental health, social drivers (determinants) of health, providing crisis and safety planning for urgent needs. Demonstrates a family-centered approach, understanding historical and generational trauma rooted in health disparities, using trauma-informed and anti-racist practices Commitment and practice in cultural humility, diversity, inclusion and belonging within community based efforts Demonstrates proficiency in documenting in care management technology platforms Flexibility to adapt to the changing/responsive nature of the Project Longecity program. Knowledge of Corrections and Law Enforcement with understanding of terminology, principles, procedures, functions and interrelationships of the criminal justice system.

Requirements

  • Degree in a behavioral health or criminal justice field preferred (three years related)
  • Experience working in an in-home setting, criminal justice and behavioral health
  • Knowledge of social services, neighborhoods, political landscape and community stakeholders throughout greater New Haven
  • Familiarity with the New Haven community and New Haven area resources
  • Strong collaboration, organization and community engagement
  • Proven ability in care coordination, case management, working with children and families
  • Proven ability in complex problem-solving and critical thinking skills
  • Comfortable with technology, provider referral and member data systems; experience with electronic health/medical records a plus
  • Team Management and/or Group Facilitation experience
  • Documentation and Reporting experience; Tableau experience a plus
  • Adaptability with cross-cultural and bilingual/multilingual community; bilingual preferred

Responsibilities

  • Required to work 3 evenings until closing and including Saturdays to meet program needs and productivity.
  • Carry a 30 person/family case load at all times
  • Ensuring completion of prescribed forms following standardized procedures and timelines for program referrals
  • Acting as liaison with Project Longevity Program Director, Program Manager, Case Manager, Outreach Workers, law enforcement partners, and other community agencies that provide consultation regarding Project Longevity involved members.
  • Attend training and utilize the Wraparound approach to work with assigned families; in-home and in the community.
  • Coordinate case conferences with the referring Project Longevity contact.
  • Participate in scheduled meetings with assigned Project Longevity Case Manager and/or Outreach Worker for quality assurance, monitoring and coaching, community education, training and education, feedback, and reporting.
  • Obtain Wraparound Certification in training modules in order to uphold fidelity to the Wraparound model.
  • Participating in and presence at Project Longevity Call-ins and other community meetings and events related to Project Longevity.
  • Ensuring and maintaining development of a balanced Family Team Meeting.
  • Providing direct care to adults, children and families as needed.
  • Participating in community and state training activities to promote individual professional growth and development around Wraparound Model and other Project Longevity initiatives.
  • Utilize selected technology platforms for conducting needs assessment, care planning development, referrals and documentation.
  • Maintain policies and standards for data management, privacy and confidentiality, HIPAA, security, and reporting as required to maintain compliance.
  • Conduct outreach and engagement activities for Project Longevity member/family including in the members home and settings in the community
  • Develop, coordinate and manage family-centered Care Plan and Care Team Meetings with members/family using the Intensive Care Coordination model for Project Longevity.
  • Maintain a trauma-informed, anti-racist, equity approach to improve the health and well-being of children, families, neighborhoods and community.
  • Utilize Electronic Health Records to complete all necessary documentation written in a timely manner consistent with all standards and regulations
  • Participate monthly program utilization meetings with the Project Longevity Program Director, Program Manager Case Manager and Outreach workers to review referrals.
  • Coordinate and organize quarterly meetings with additional Project Longevity program partners to discuss any gaps in services.
  • Performing other related duties as directed.
