About The Position

The Case Manager Registered Nurse is a professional member of the hospice interdisciplinary team who provides skilled nursing care to patients in compliance with the Nurse Practice Act, the regulations of the State Board of Nursing and the policies and procedures of Klamath Hospice, Inc. A Case Manager is responsible for planning, coordinating, providing and documenting care for terminally ill patients from day of admission through day of discharge. Availability for weekends a plus! Requirements

Graduate of an accredited school of nursing
Current RN licensure in the state of Oregon, with ability to obtain licensure in state of California
Experience as an RN commensurate with ONE of the following: one year acute care experience within the last three years; one year home health care experience within the last three years; one year community health or public health experience within the last three years; one year hospice experience within the last three years; or, have a baccalaureate degree in nursing from a program accredited by the National League of Nursing.
Demonstrated leadership capabilities with the ability to supervise staff and other professionals
Basic computer knowledge. Ability to obtain proficiency in EMR program.
Comprehensive knowledge of general nursing theory and practice
Current BCLS certification
Possess valid state driver's license and reliable automobile and be willing to operate personal car necessitated by nature of job
Proof of current automobile insurance with minimum state financial responsibility limits
Preferred applicant will be required to pass a background check and pre-employment drug screen, following a job offer.
