About The Position

The Case Manager full-time position is responsible for providing comprehensive supportive services to all Charm City Care Connection participants. Services provided by the Case Manager include assessment of participant needs and goals, case planning, referrals, support with accessing benefits, ID documents, medical care, substance use treatment and providing warm handoffs to any other outside resources. Most importantly, this position builds long term partnerships with our participants in an effort to support them in reaching their goals. This position reports to the Manager of Supportive Services.

Requirements

  • Two years of case management experience or significant work-related or life experience
  • We are specifically looking for candidates with an active interest in harm reduction and/or social justice as it pertains to individuals who use drugs and communities impacted by the war on drugs
  • Experience with individuals experiencing homelessness, mental illness, and substance use
  • Proficiency in computer packages including MS Office applications, and willingness to learn and use other electronic data collection and case manager computer applications is required
  • Flexible and able to work through a variety of case concerns
  • Familiarity with Baltimore health, housing, and social service providers
  • Must be committed to working with people who use drugs in a way that affirms dignity and humanity
  • Ability to work with diverse communities (gender, ethnic, age, language, political, sexual orientation)
  • Ability to work in an active and busy drop-in center and help manage the milieu
  • Excellent communication skills (writing and oral); timely follow up on tasks for staff and supervisor
  • Must have a valid driver’s license and reliable transportation

Nice To Haves

  • Training in human services such as social work, psychology, case management, or counseling is strongly preferred.
  • Must be an organized, self-motivated team-player, with strong interpersonal and communication skills, and the ability and willingness to be hands-on and solve problems as they arise
  • Great attitude and people skills are a must
  • Background in harm reduction models a plus
  • Experience with housing resource navigation & knowledge of housing models preferred
  • Ability to master a wide range of information and communicate accordingly
  • Self-motivated to identify and attend professional development training opportunities

Responsibilities

  • Identify participant needs, establish goals, and develop care plans.
  • Connect participants with the appropriate services to meet their needs and achieve goals. This includes incremental goals such as safer injection practices or obtaining stable housing.
  • Provide direct case management services, coordinating referrals for essential needs: public assistance, food/shelter, medical care, transportation, and mental health/substance use support.
  • Connect participants to essential services like medication-assisted treatment (MAT)
  • Continuously follow up with participants to support progress, monitor outcomes, and proactively remove barriers to service access and goal attainment.
  • Complete program documentation, including compiling reports and maintaining both written and computerized case records.
  • Evaluate clients' substance use, sexual health, and immediate safety risks (e.g., overdose potential) in a non-judgmental manner.
  • Liaise with and accompany clients to healthcare and social service agencies (e.g., Social Services, Vital Records, MVA, medical appointments) to help them navigate complex systems.
  • Continuously develop and maintain current knowledge of local service programs and resources, taking initiative to seek out and learn about new programs and changes to existing programs and services
  • Regularly check-in with outside referral programs to ensure services remain accessible and of good quality
  • Maintain a presence during designated drop-in hours to readily engage with individuals interested in case management services.
  • Must be open to engaging participants on the spot and working toward their immediate goals
  • Provide administrative support and oversight to new and ongoing programming
  • Connect participants to harm reduction education and resources when appropriate
  • Participate in staff meetings and professional development
  • Other duties as assigned by the supervisor
