About The Position

InterCare is searching for a full-time Case Worker to join our Team in Holland! Requirements

  • Professional level knowledge typically associated with a Bachelor’s level degree in a health and human services field, such as, social work, social services, psychology, counseling/education, sociology, or public health.
  • One year of case management/case work experience or healthcare related experience.
  • Ability to work effectively with a wide variety of people and develop and maintain professional working relationships with site and administrative staff.
  • Possesses skills in motivation, self-direction, and punctuality.
  • Ability to work well as part of a team and keep team members updated on current project developments.
  • Remains flexible and calm in changing circumstances and stressful environments, while performing multiple priorities.
  • Ability to think creatively and strategically, organize time efficiently, remains attentive to detail and identify necessary resources.
  • Contributes to a positive and respectful workplace.
  • Knows when to and seeks out advisory guidance as needed.
  • Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature.
  • Consistently exercises discretion and judgment to analyze, interpret, make deductions and then decide what actions are necessary based on the varying facts and circumstances of each individual case.
  • Maintains a working knowledge of the Primary Care Team’s roles and responsibilities.

  • Understands and consistently supports the uniform implementation of InterCare’s organizational and patient care policies and procedures as pertinent.
  • Maintains BLS requirement. BLS is a mandatory by InterCare and our FTCA agreement with the Federal Government. It must be completed within 90 days of hire.
  • Proficient in Microsoft Office Suite. Has ability to create letters, forms, documents, presentations.
  • Proficient in eCW.
  • Proficient in InterCare’s clinical competency requirements.
  • Possesses a professional level of written and verbal communication skills.
  • Ability to communicate complex concepts in a clear effective manner.
  • Possesses excellent cross cultural communication skills and the ability to communicate professionally to patients, providers, individuals in the community and or representing to outside organizations/businesses.
  • Ability to comfortably speak and present in a front of groups.
  • Ability to stand, walk, stoop, kneel, crouch, frequently sit, and lift up to 25 lbs.
  • Requires good hand-eye coordination, reach with arms & hands, and finger dexterity, including ability to grasp, and manipulate (using keyboard, office equipment).
  • Visual acuity to use keyboard, operate office equipment, and read printed material and regularly required to talk and hear.

Responsibilities

  • Provides supportive case management services to the patient based on individual need.
  • Consults and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining treatment goals.
  • Works with the patients on a day-to-day basis using professional judgment and discretion to implement the team determined Treatment Plan.
  • Assists patient in setting up medical/behavioral health appointments and ensuring the patient keeps appointment.
  • Provides crisis management for clients; makes linkages and referrals for interventions as appropriate.
  • Helps patient obtain necessary resources and advocates for the patient with special needs.
  • Assists community members and health field personnel to assess, plan for, and provide needed health and related services such as food, housing/shelter, insurance/benefits, mental health treatment, counseling, crisis support; including support groups, education, employment, disability insurance and transportation needs.
  • Refers patients needing additional support of social and emotional problems to other community agencies such as Community Mental Health for further assistance.
  • Links and monitors patient activity related to counseling and treatment plan.
  • Completes all of the steps assigned to Case worker, regarding the SWMBH SUD and Duals/Ottawa CMH SUD Billing process, in a timely manner.
  • Other duties as assigned.
