Requirements

  • High school diploma or equivalent required.
  • One year of general office experience, preferably in home health or healthcare.
  • Strong organizational skills, attention to detail, ability to manage multiple priorities, and maintain accuracy under pressure.
  • Demonstrate ability to work independently with alertness and precision in a high-volume, time-sensitive environment.
  • Proficiency with Microsoft Office applications, computer skills, and electronic health records (HER) systems.

Nice To Haves

  • Experience with Homecare Homebase (HCHB) and Mosai is preferred.

Responsibilities

  • Receive and respond promptly to referral requests via email, fax, and electronic portals.
  • Oversee the Referral Bot activity via the Bot Dashboard, Teams chats, email, electronic portals, etc.
  • Creates and maintains an organized system that allows for quick and accurate responses to emails and other requests related to the referral intake process.
  • Prioritize and manage multiple referrals simultaneously to ensure timely response and processing.
  • Deliver exceptional customer service through timely, clear, and compassionate communication to patients, staff members, other healthcare professionals and referral sources.
  • Responds to agency and patient needs in a professional and creative manner.
  • Support continuous improvement efforts by identifying process gaps and suggesting workflow enhancements.
  • Demonstrates commitment, professional growth, accountability, and competency.
  • Promotes the company philosophy and administrative policies to ensure quality of care.
  • Cross train as a Centralized Document Indexer and Centralized Intake Coordinator.
  • Interact with team members and co-workers in a positive and professional manner.
  • Maintains contact/communication with all personnel involved in the referral intake process to promote coordinated and efficient care.
  • Participate in agency-sponsored in-service training.
  • Participates in personal and professional growth and development.
  • Participate in On-Call and/or weekend coverage, as needed.
  • Performs other duties as assigned.
