Position Overview: As a Hospice CNA, you will offer comfort and support to our patients and their loved ones. You will work alongside a team of healthcare professionals to provide personalized care, emotional support, and physical assistance to ensure that our patients' physical and emotional needs are met. Your compassionate companionship will make a difference in the lives of those you care for. This position is a home care position. Monday-Friday. 8AM-5PM

Requirements

  • Valid certification as a Certified Nursing Assistant (CNA) in Colorado.
  • Valid non-restricted Drivers License
  • Compassionate demeanor and a genuine desire to make a difference.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively both independently and as part of a team.
  • Empathy, patience, and a nonjudgmental attitude toward patients and families.

Nice To Haves

  • Prior experience in hospice care is preferred but not required.

Responsibilities

  • Provide compassionate, holistic care to hospice patients in their homes or residential facilities.
  • Assist patients with activities of daily living, including bathing, grooming, and toileting.
  • Monitor and report changes in patient's condition to the interdisciplinary team.
  • Offer emotional support and companionship to patients and their families.
  • Help maintain a clean, safe, and comfortable environment for patients.
  • Collaborate with nurses, physicians, social workers, and other team members to ensure coordinated care.
