About The Position

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers. The Certified Nurse Assistant (CNA) is an essential member of the hospice care team who works under the supervisor of a Registered Nurse (RN) within Bluegrass Care Navigators’ Hospice Inpatient Units. This role provides personal care and support services to meet the patient’s daily needs, enhance comfort, and maintain a safe environment. The CNA is also responsible for observing the patient’s condition and accurately reporting and documenting observations and the services provided.

Requirements

  • At least 18 years of age and a high school graduate or equivalent.
  • Currently registered as a State Nurse Aide with the Kentucky Board of Nursing.
  • Completion of a recognized Certified Nurse Assistant (CNA) training program.
  • Ability to read, understand, and follow written instructions, and accurately document care provided.
  • Self-directed with the ability to work independently with minimal supervision.
  • Demonstrates empathy for individuals who are ill, frail, or impaired, and understands the philosophy of hospice care and the needs of the terminally ill.

Nice To Haves

  • One year of experience in a hospital or nursing home setting, preferred.
