About The Position

Traditions Home Health Services is currently seeking compassionate, skilled, reliable, and experienced Home Health Aide and/or CNA for the Plymouth area to assist with our senior clients who are in need of support with Activities of daily living, household support and companionship. We are a trusted provider for the Veterans Administration, are you able to help out with our great Veteran client's that have done so much for our Country? Shifts Available: Daily 4-Hour Shifts example: 8am-12pm, 9am-1pm, 10am-2pm ect... 3pm-7pm, 4pm-8pm Day shifts: 8am-8pm, 10am-4pm (client states what hours are needed) Job Description: CNA or HHA would assist with the Activities of Living, personal care CNA or HHA would assist with light housekeeping CNA or HHA would encourage walking, exercising CNA or HHA would encourage stimulating conversation CNA or HHA would assist Activities and exercise according to the Care Plan CNA or HHA would encourage fluid intake, possible meal prep in accordance with orders/Care Plan Home Health Aide or Certified Nursing Assistant would competently documenting shift activities frequently during the shift digitally using a smartphone Home Health Aide or Certified Nursing Assistant would report any concerns to the Traditions Home Health Services Office Support Team in a timely manner Traditions Home Health Services are in high, high demand! Requirements

  • Home Health Aide Certification or CNA Certification
  • 2+Years experience working with your Certification
  • Must be kind, compassionate, reliable, and skilled HHA
  • Must have experience working with Dementia
  • Clean driving record, ability to pass a CORI check
  • Current TB test, QuantiFERON Gold Test, or X-ray
  • Dementia care experience
  • Speak, read, write the English language clearly, and effective communication skills.
  • You must own a cell phone.

Responsibilities

  • Assist with Activities of Daily Living
