About The Position

With a commitment to patient- and family-centered care, the professional nurse uses the nursing process in ways that values the uniqueness of each individual and addresses the physical, psychological, emotional, and social needs of the diversity of patients served by Jefferson Health. Assists in leading the daily operations of a specific unit. Acts as a resource, lead, and clinical role model for the patient care team. Advances and oversees patient care standards of the unit for the designated shift. Also facilitates and coordinates patient care delivery and flow through the continuum of care. Supports nursing professional development, Magnet, and Shared Governance through participation in, and mentoring of colleagues, on councils and the Clinical Ladder program

Requirements

  • BSN Degree Required; or to be completed within 3 years of hire
  • State Issued RN PA Licensure required (MSL out of DE or NJ)
  • Basic Life Support (BLS) Certification Required
  • Minimum 3 years RN ER/Critical Care Experience Required

  • Patient-and Family-Centered Practice- Knowledge of how to convey respect and collaborate with each patient, family and/or designated care partner in care.
  • Cultural Awareness- Knowledge of people with diverse backgrounds and the ability to convey respect and connect effectively.
  • Care Coordination Knowledge of the medical, social, economic, and other services provided by other departments or facilities, and the ability to integrate these multidisciplinary treatments for optimal patient care.
  • Patient Safety Knowledge of the risks faced by patients in a healthcare environment, and the ability to take steps to reduce these risks.
  • Medication Administration and Prescription Knowledge of various medications used to treat patients, ability to apply this knowledge to the administration of medications and to the prescription of drugs for continuing treatment outside of the hospital.
  • Discharge Planning Knowledge of the patient's post-discharge needs and the ability to consider these needs in planning for discharge and arranging for follow-up care.
  • Assessment and Diagnosis Knowledge of various tools and techniques for patient assessment and nursing diagnosis or clinical judgement, ability to explore and analyze reasons behind symptoms and generate resolutions according to available drugs, facilities and techniques.
  • Health Information Documentation Knowledge of the relevance of health information and appropriate documentation methods, ability to record medical histories, symptoms, vital signs, medications, diagnoses, treatment plans, procedures, tests, and medical results in written or electronic form.

Responsibilities

  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Recognizes the patients, their family/designated care partner(s) in providing safe, compassionate and coordinated care based on respect for patient's personal preferences, priorities, needs, and cultural beliefs and values.
  • Communicates with and supports the interprofessional teams and services, patient, family and designated care partners.
  • Serves as a clinical resource regarding policies and procedures, patient standards, performance improvement, and risk management issues.
  • Collaborates with the medical staff to facilitate consistency of interdisciplinary plans of care to ensure quality patient care.
  • Assesses patient needs and makes care recommendations based on the patient’s physical, psychosocial and cultural needs, age and functional status.
  • Facilitates timely patient throughput optimization for all phases of care including admissions, transfers, and discharge optimization of patients on the designated unit.
  • May participate in plan of care rounds depending on service and unit protocols.
  • Provides timely Smart Square schedules and on call schedules if applicable.
  • Follows up appropriately and effectively resolves customer and physician issues and complaints.
  • Monitors staff schedules to ensure adequate staffing to meet patient care needs and maintain quality patient care.
  • May authorize overtime or scheduling changes to fill shifts as necessary.
  • Completes payroll in collaboration with Nurse Manager.
  • Provides volumes & adjustments to reflect variances in staffing needs within Smart Square.
  • Collaborates with the Nurse Manager to evaluate, maintain and improve patient care team member performance.
  • Assists in implementing changes in clinical practice and evaluates the quality and effectiveness of nursing practice to ensure compliance with organization, department and unit specific goals.
  • Supports nursing professional development, Magnet, and Shared Governance through participation in, and mentoring of colleagues, on councils and the Clinical Ladder program.
  • Provides patient care support when necessary.
