About The Position

FirstLight Home Care of St. Paul provides compassionate, high-quality in-home care for seniors and adults who wish to remain safely and independently in their homes. We focus on clinical quality, clear communication, and strong community relationships. We are seeking a Client Care Coordinator (CNA Required) to lead client intake and visitation, support ongoing care coordination, and participate in community outreach and networking. This role is ideal for a CNA who is comfortable conducting in-home visits, understands personal care needs, and can confidently represent our agency to clients, families, and referral partners. Caregiver scheduling is not part of this role.

Requirements

  • Active CNA certification (required)
  • 3+ year experience as a CNA or in-home care professional
  • Comfortable conducting in-home client visits independently
  • Strong communication, assessment, and documentation skills
  • Reliable transportation and willingness to travel locally
  • Organized, professional, and empathetic

Nice To Haves

  • Home care or hospice agency experience
  • Intake, admissions, or care coordination experience
  • Networking or relationship-based sales experience
  • Bilingual (a plus)

Responsibilities

  • Conduct initial in-home client visits and intake assessments
  • Gather client history, care needs, preferences, and safety information
  • Explain services, care process, and next steps to clients and families
  • Coordinate intake findings with the RN and management team
  • Serve as a primary point of contact for clients and families after intake
  • Monitor client satisfaction and address non-clinical concerns
  • Identify care changes or risks and escalate to the RN as needed
  • Support understanding and adherence to care plans
  • Build and maintain relationships with referral sources (hospitals, rehab centers, senior living communities, social workers, churches, and community organizations)
  • Participate in networking events, tours, and referral meetings
  • Follow up on inquiries, leads, and referrals in a timely manner
  • Represent FirstLight Home Care professionally in the community
  • Complete accurate intake documentation and visit notes
  • Maintain records in care management and CRM systems
  • Support onboarding of new clients from intake to start of care
