About The Position

Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care. The Clinic and Nursing Manager is responsible for the coordination and implementation of health services in the clinic and community, including assessments, treatments, procedures, care plans, order tracking, and lifeline contract oversight. Ensuring coordination of care between PACE and contracted facilities, vendors, and providers. Responsible for departmental compliance with state and federal regulations, participation in organizational Quality Assurance program, departmental training, education, and coordination of provision of program services as part of the PACE IDT leadership structure.

Requirements

  • Associate Degree in Nursing.
  • License to Hold the Position Registered Nurse (RN).
  • 2-3 years relevant experience in nursing management.
  • Must possess BLS/CPR certification or be able to obtain within 60 days of hire.
  • Experience working within an Interdisciplinary Team Model.
  • Experience with Electronic Medical Records (EMR).

Nice To Haves

  • Experience working with individuals with mental health needs (Preferred).
  • Experience working with the frail or elderly population (Preferred).
  • Experience in team management, preferably in a geriatric care setting.

Responsibilities

  • Demonstrates effective leadership in managing daily department operations, managing staff, planning, and evaluating department activities to accomplish department objectives.
  • Develops, coordinates, and monitors department practice, standards, policies, and procedures based on established industry knowledge and research in accordance with all applicable federal, state, local, professional, and regulatory requirements.
  • Defines department quality objectives, identifies, and evaluates trends, implements courses of action, and evaluates outcomes.
  • Collaborates with the TRU PACE Operations Director in the annual and capital budget development and monitors departmental expenditures and maintains the departmental budget within its allowed and reasonable objectives as well as meet established budgetary guidelines.
  • Reviews and resolve quality issues and grievances timely in collaboration with QA department and Operations Director, recommends and implements system improvements as needed.
  • Ensures job responsibilities, authorities, and accountability of direct reports are defined and understood.
  • Accomplishes department objectives by communicating job expectations, planning, monitoring, and appraising outcomes.
  • Interviews, hires, trains, coaches, and provides constructive feedback and training opportunities to employees. Conducts employee disciplinary action in collaboration with supervisor and HR as needed.
  • Ensures completion initial and annual competencies timely, and provides direct oversight, to all department employees, contracted providers and others as assigned.
  • Develops annual department and individual employee goals, assesses employee performance, and develops employee performance improvement plans in conjunction with HR when needed.
  • Develops personal growth opportunities and facilitates team building for direct reports.
  • Holds employees accountable to company quality and customer service standards.
  • Participates as an active member of PACE IDT and TRU Community Care Leadership groups.
  • Participates as a member of the interdisciplinary team (IDT). Maintains regular attendance at, and participates in all IDT meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24-hour care delivery.
  • Participates in the PACE QAPI program by identifying systemic opportunities for improvement, taking leadership of critical quality improvement initiatives, and contributing to process improvement projects.
  • Effectively identifies, responds to, recommends and/or resolves problems as they arise in collaboration with leadership team and IDT.
  • Takes lead in effectively communicating with IDT and community entities at meetings and throughout the day as needed.
  • Communicates effectively with the Operations Director and Medical Director to keep them abreast of any issues or concerns.
  • Develops and maintains effective work relationships with participants, families, peers, co-workers, contracted entities and vendors, and the public.
  • Protects privacy and maintains confidentiality of all company procedures and information about employees, participants, and families.
  • Follows all PACE Program Policies and Procedures and OSHA Safety Guidelines.
  • Participates in the on-call process. Communicates with weekend and after-hours staff, following up appropriately.
  • Ensures that the Clinic and Homecare staff works within their scope of practice.
  • Manages flow of Clinic and Homecare nursing services, oversees appointment bookings, triaging and gate keeping in conjunction with primary care, RN Case Managers, and staff. Ensures timely and appropriate care of participants.
  • Oversees and/or performs initial assessments of participants in the clinic and in the home environment to determine their individual needs.
  • Coordinates with the interdisciplinary team to develop care plan for each participant and coordinate caregiving resources as needed.
  • Oversees or performs annual re-assessments of all participants to determine that appropriate clinic and homecare services are provided.
  • Ensures homecare nurses perform supervisory visits with CNAs in the field as required by applicable laws and practice standards.
  • Ensures skilled supportive and restorative nursing care is provided to participants as ordered based on the plan of care.
  • Oversees and ensures signing of orders, ordering medications from pharmacy, and disseminating orders and instructions to appropriate personnel. Ensures implementation of physician orders.
  • Communicates with weekend and after-hours staff, following up appropriately.
  • Manages and reviews participants’ records, charts, pertinent information, and staff documentation.
  • Documents as needed in medical records according to PACE program standards.
  • Performs general skilled nursing care to participants. Performs or supervises the administration of prescribed medications and treatments in accordance with nursing standards. Assist in treatment, examination, and testing of participants.
  • Responds to and performs triage of incoming telephone calls. Oversees scheduling of appointments for participants when necessary.
  • Assists with aspects of hospital, rehabilitation, and nursing home admissions.
  • Coordinates participant care with outside contracted service providers – nursing homes, assisted living facilities, lab, medical equipment, etc.
  • Educates participants, family and caregivers in care, medications, treatments, and all other aspects of health education as needed.
  • Educates staff in care and treatment of the frail elderly as needed.
  • Handles potentially infectious specimens with established biohazard precautions and practice standard precautions.
  • Ensure insurance authorization processes are followed effectively to sure prompt services and payment.
  • Ensures orders tracking processes are followed effectively and efficiently to sure quality of care.
  • Ensure adequate clinic and medical supplies and ordering as needed.
  • Review notes and create orders for footcare/scan to medical records/
  • Retrieve labs/vital signs/medical information from Point Click Care (SNF).
  • Assign PCP/RN Case managers and Schedule new enrollees for clinic assessments.
  • Ensure homecare nurses are scheduled for home visits for new enrollees.
  • Ensure care plans are initiated and evaluated for services provided by home care nurses and the contracted home care agencies and PACE CNAs.
  • Block and schedule PCPs calendar for meetings.
  • Create and block schedule for On Call nursing rotations.
  • Review and monitor tasks in EMR.
  • Meet regularly with home care nurses to review current goals and priorities for the week.
  • Ensure orders entered by the PCP’s are effectuated timely by all clinic and home care staff.
  • Oversee and ensure collaboration between the homecare nurses and the clinic for labs, wound care, and triage visits.
  • Oversee the requesting and monitoring accounts with Connect America for lifeline devices.
  • Consulting with other departments and other PACE programs as needed to help develop and fine tune processes and assist with strategies and approaches to improve efficiency and reduce redundancy.
