To learn more: https://www.wcbay.com/services/birdsong-clinic/ The Clinic Licensed Practical Nurse (LPN) Provides nursing and clinical intervention and medication administration for assigned residents on a nursing unit and ensures smooth operations of the unit for a designated shift. Administers medications for the unit(s) assigned in compliance with state regulations and pharmacy regulations. Familiarizes his/herself with each medication administered, observes and stays aware for resident response to the medications, knows the optimal time for the medication, and confers with the physician about the medication administration. Requirements

  • Active, unencumbered LPN licensure in the state of Virginia (or compact state).
  • Current CPR certification.
  • Minimum of one year of experience in a healthcare setting.
  • Flexibility to work varied schedules.
  • Flu-Vaccinated

Nice To Haves

  • Prior experience working with the elderly population.
  • Experience working with residents who exhibit challenging behaviors.
  • COVID-19 Vaccinated

Responsibilities

  • Perform blood glucose monitoring, inhalation treatments, and other physical prescribed medical treatments/diagnostic test.
  • Maintain medication and resident’s records, responsible for electronic input of data in EHR system.
  • Transmit requests for medication refill per policy to ensure proper medication administration to assigned residents.
  • Participate in resident assessment and care planning reviews and revise assessments and care plans as needed.
  • Ensure problems and complaints involving nursing services are reported in a timely manner.
  • Communicate and coordinate care with residents and their families.
  • Inspect the environment, identify environmental problems and initiate requests for correction.
  • Identify, report and document incidents and participate in investigation of incidents and as necessary.
  • Identify clinical and operational problems, and works with a team to implement resolution.
  • Review and transcribe physician orders; Communicate with residents’ physicians as directed.
  • Implement, practice and assure staff adhere to infection control measures.
  • Represent the community and nursing services in professional activities as assigned.
  • Support and participate in community and departmental quality assurance programs.
  • Ensure that unit practices are consistent with regulatory requirements.
  • Maintain positive working relationships between nursing staff and other departments.
  • All other duties as assigned.
