Clinical Ambulatory Pharmacist | Specialty Clinic
About The Position
CentraCare is seeking a Clinical Ambulatory Pharmacist to join our Medication Therapy Management (MTM) team and help improve the health of rural communities. In this role, you will provide comprehensive medication management services to ensure each patient’s medication therapy is indicated, safe, effective, and convenient. You’ll work directly with patients while collaborating with an interprofessional care team to optimize medication use and improve health outcomes. This position is ideal for a pharmacist who brings strong clinical expertise and values building positive, trusting relationships with both patients and colleagues across the care continuum.
Requirements
- PharmD degree and/or BS Pharm degree required.
- Current licensure by the Minnesota State Board of Pharmacy or eligibility for licensure required.
- Basic Life Support (BLS) Certification
Nice To Haves
- Completion of a PGY1 or PGY2 ambulatory care residency or comparable experience in ambulatory clinical pharmacy highly preferred
- Eligible for Pharmacy Specialty Board Certification preferred
