About The Position

The Clinical Care Coordinator III demonstrate the competency to provide patient care according to the age-specific population needs and assist in the management of patient care in the physician office setting under the direct supervision of a provider.

Requirements

• Basic Life Support/CPR certification; or must be obtained within thirty (30) days of hire/transfer date.
• Graduate of an accredited school of nursing or a diploma program.
• A minimum of one (1) year of clinical practice experience in a medical/physician office setting.

Nice To Haves

• Electronic Health Records experience.
• Demonstrated ability in establishing and maintaining effective working relationships.
• Strong oral and written communication skills.
• The ability to perform multiple tasks simultaneously, while maintaining proficiency and efficiency.

Responsibilities

• Perform comprehensive intake reviews for new obstetric (OB) patients entering the practice, utilizing the Antenatal Risk Score as outlined in the study Association of Antenatal Risk Score With Maternal and Neonatal Mortality and Morbidity to identify risk factors and support appropriate care planning for maternal and neonatal outcomes.
