About The Position

To supervise and manage operations of the Population Health department.

Requirements

  • Minimum BSN or BA education required.
  • Professional knowledge of business and management theory and practice at a level normally acquired through completion of an Associate’s Degree.
  • Computer skills are required.
  • Experience with Microsoft Office desirable.
  • Minimum five years progressively responsible management experience in healthcare or customer-oriented service industry.
  • Verbal and written skills necessary to effectively communicate with various members of the healthcare team, other health facilities, community health related organizations, various external parties and regulatory agencies.
  • Ability to manage numerous management issues.
  • Must be able to use the following management accountabilities and competencies: conflict resolution, staff counseling, problem solving, disciplinary process, policy and procedures implementation, team work, staff scheduling, interviewing, budgeting process, cost containment skills and support of management peers.

Responsibilities

  • Effectively communicates with MMC departments to ensure an effective continuum of care for patients.
  • Regular communication with Pop Health Director.
  • Coordinate clinical team to recognize and resolve care gaps.
  • Supervises staff.
  • Coordinate and assist with clinical recruitment, orientation/training.
  • Conduct clinical performance appraisals.
  • Reviews schedules for effective clinical coordination.
  • Works with assigned clinical Works with assigned team members to provide guidance for Allegheny Health Coaches.
  • Supervises scheduling of care coordination.
  • Supervises scheduling for patients and community interventions.
  • Ensures accuracy of patient coordination.
  • Reviews schedules for productivity.
  • Create and maintain schedules for clinicians.
  • Works with Social Worker to effectively coordinate community resources.
  • Responsible for acquiring knowledge of ACO reporting requirements for compliance and program management.
  • Periodically reviews CMS requirements for wellness visits and documentation requirements.
  • Performs audit checks for appropriate and compliant charting requirements.
  • Provides education and guidance for medical management of chronic disease.
  • Performs regular chart review for clinical appropriateness.
  • Works with administrator to establish patient selection criteria.
  • Acts as liaison with MMC for appropriate patient selection.
  • Coordinates health professionals for effective patient management.
  • Reviews requirements for CCM and PCM documentation, billing, and outcomes for individualized treatment care plans.
  • Communicates with CORE team for patient progress.
  • Developing and Implementing of Program Orientation/ Training.
  • Developing appropriate integrative Protocols, Quality Metrics, Tracking and Auditing tools.
