About The Position

The Clinical Director of Home Health provides oversight and coordination of the clinical and operational divisions of the home health agency. This role manages all patient care services and personnel including nursing, home health aides, medical social work, physical therapy, occupational therapy, and speech-language pathology staff and contractors. The Clinical Director ensures that patient care is coordinated effectively through appropriate assignments, referrals, and the development and implementation of individualized plans of care. The Clinical Director assists with oversight of safety and infection control programs, chart audits, and medical supplies while ensuring that medical records are maintained appropriately. This role assures that all clinical disciplines function according to licensure requirements, state law, and Medicare/Medicaid standards for Home Health services. The Clinical Director works closely with the Administrator to support the overall operation of the agency and may act on behalf of the Administrator when authorized. Home Health care focuses on delivering skilled nursing and therapy services to patients in their place of residence. The goal of home health services is to promote independence, recovery, and improved health outcomes while allowing patients to remain safely in their homes. Requirements

  • Graduate of an accredited nursing program or licensed physical therapy program.
  • Licensed Registered Nurse or Physical Therapist in the State of Ohio.
  • Valid Ohio driver’s license and reliable transportation that is insured according to state requirements.
  • Minimum three (3) years of clinical experience, with at least one (1) year in home health required.
  • Minimum two (2) years of supervisory or management experience.

Responsibilities

  • Provides direction, supervision and consultation to the clinical staff.
  • Clinically manages and oversees patient care by coordinating patient assignments, patient care services, and referrals
  • Ensures patient needs are continually assessed and that individualized plans of care are developed, implemented and updated appropriately
  • Serves as a resource to all agency personnel regarding clinical and operational activies
  • Oversees the provision of clinical care including nursing, home health aide, medical social work and therapy services consistent with stands of practice, agency policies, Medicare/Medicaid guidelines and state licensure laws.
  • Develops written goals, objectives and activities for the delivery of home health services.
  • Participates in program planning including identifying new service areas and evaluating the effectiveness of clinical programs.
  • Works with clinical staff to assist with complex patient care issues and adjustments to patient care plans as needed.
  • Collaborates with clinicians and physicians to assess patient situations, coordinate care and support appropriate referrals.
  • Ensures each patient receives quality care consistent with agency policies and regulatory guidelines.
  • Utilizes clinical knowledge and skills to oversee the provision of patient care consistent with the mission of Otterbein Home Health.
  • Participates in Quality Assessment and Performance Improvement (QAPI) activities and Outcomes Based Quality Indicator (OBQI) initiatives.
  • Develops and maintains clinical policy and procedure manuals.
  • Oversees proper maintenance and review of clinical records.
  • Oversees the planning and evaluation of infection control and safety programs.
  • In the absence of the Administrator, may assume responsibilities of the Administrator as authorized.
  • Assists with recruiting, hiring and managing home health employees and contract personnel.
  • Works with the Administrator to determine staffing patterns that promote effective utilization of personnel.
  • Identifies staff development needs and promotes continuing education and in-service training opportunities.
  • Conducts employee performance evaluations following the probationary period and annually thereafter.
  • Ensures accurate and timely completion of payroll reports, time sheets, incident reports and required personnel documentation.
  • Communicates patient care concerns, staffing challenges or personnel issues to the Administrator in a timely manner.
  • Ensures that home health nurses, aides and contracted personnel receive appropriate supervision.
  • Ensures services are coordinated and available through Home Health Care RNs 24 hours a day, 7 days per week.
  • Maintains appropriate levels of medical supplies necessary for patient care.
  • Provides supervision and coordination of therapy services and ensures peer supervision when appropriate.
  • Demonstrates an understanding of staff productivity and effective contractor utilization.
  • Utilizes available data and reports to coordinate staffing assignments and patient visits.
  • Participates in budget preparation and census and visit projections for clinical services.
  • Evaluates services through patient outcomes, statistical reports and quality improvement activities.
  • Assists with investigating patient or customer concerns and complaints.
  • Approves requests for time off while ensuring adequate coverage for all clinical services.
  • Manages staffing to align with census needs and referral activity.
  • Works with the Administrator to recruit, train and retain staff based on agency and community needs.
  • Ensures clinicians operate within established caseload guidelines and productivity standards.
  • Maintains current knowledge of home health principles and practices.
  • Maintains knowledge of discipline practice acts, regulatory requirements, and professional standards.
  • Develops and maintains cooperative relationships with referral sources, healthcare providers and community organizations.
  • Represents Otterbein Home Health in professional and community settings as appropriate.
  • Exercises independent judgment and critical thinking to address patient care and operational challenges.
  • Effectively manages competing demands and prioritizes responsibilities.
  • Functions effectively as a collaborative member of the healthcare team.
  • Maintains confidentiality of patient and agency information in accordance with HIPAA guidelines.

  • Ensures accurate and appropriate maintenance of clinical records in accordance with agency policies.
  • Coordinates with referral sources to maintain a consistent patient census.
  • Ensures staff productivity aligns with agency operational and regulatory expectations.
  • Ensures compliance with all federal, state and local regulations including Medicare Conditions of Participation for Home Health agencies.
  • Conducts all business activities in a professional and ethical manner.
