About The Position

The Behavioral Health (BH) Clinical Care Manager utilizes advanced clinical judgment and critical thinking skills to facilitate appropriate physical and behavioral healthcare and social services for members through assessment and member-centered care planning, direct provider coordination/collaboration, and coordination of psychosocial wraparound services to promote effective utilization of available resources, optimal member functioning, and cost-effective outcomes.

Requirements

  • This position requires the incumbent to have a master’s degree in social work or counseling.
  • Louisiana licensure as a Licensed Mental Health Professional (e.g. LCSW, LPC, LMFT, LAC, APN, PhD, or PsyD.) required.
  • 8 years management and/or post graduate clinical practice experience (e.g., hospital setting, alternative care setting, home health or ambulatory care) required.
  • This position requires the ability to communicate clearly in person and/or via telephone communication, and in writing.
  • Requires the ability to provide services to the target population, organizational skills, and the ability to prioritize multiple tasks concurrently. Interact with appropriate agencies and persons in the community in developing services.

  • Satisfactory Motor Vehicle Report
  • Must be able to pass a criminal history investigation
  • Be able to obtain Cardiopulmonary Resuscitation and First Aid certification

Nice To Haves

  • Level of Care Utilization System (LOCUS)/CALOCUS certification, case management, discharge planning experience, managed care, and crisis intervention skills are strongly preferred.
  • Candidates without current certification will be required to obtain certification post hire.

Responsibilities

  • Evaluate and complete annual comprehensive assessments; required supervisory forms; client treatment plans every 180 days; and client crisis assessments.
  • Request ongoing authorization for Community Psychiatric Support Treatment and Psychosocial Rehabilitative Support.
  • Monitor key performance indicators; completion of 90 Day Progress Summary Report; required performance standards; and file compliance (including annual documents and performance evaluations).
  • Facilitate clinical staff meetings; team training; interviews and onboarding process for new staff; training for new hires.
  • Review and approve case notes, time sheets, and mileage reimbursement.
  • Provide training and implementation of managed care or VOASELA requirements.
  • Interface as liaison between managed care organizations; agency leadership; employees with support for professional development; and supervision to non-licensed staff.
  • Ensure written documentation is provided and recorded of supervision to non-licensed staff
  • Complete and sign agency supervision of Certified Social Workers in accordance with Act 431 and attest to agency supervision of Provisional Professionals.
  • Review and approve case notes, PSH forms, time off request, time sheets, and mileage reimbursements.
  • Manage day-to-day program operations; and manage/operate within fiscal budget.
  • Develop corrective action plans; system improvements; and develop/participate in celebration initiatives.
  • Implement professional development goals and promote sustained business growth.
  • Participate in On Call rotation.
  • Perform other duties as required.
