Lakeport Post Acute is excited to welcome a Clinical Fellowship Year (CFY) Speech-Language Pathologist to our therapy team! This is a great opportunity for a new graduate to complete their Clinical Fellowship in a supportive, collaborative skilled nursing environment while making a meaningful impact on the lives of our residents. Requirements

  • Master’s degree in Speech-Language Pathology
  • Eligible for Clinical Fellowship Year (CFY)
  • California SLP license or eligibility
  • Strong communication and organizational skills
  • Compassionate, motivated, and eager to learn
  • Interest in working in a skilled nursing/post-acute setting

Responsibilities

  • Provide speech-language therapy services under CFY supervision
  • Evaluate and treat residents with speech, language, cognitive, and swallowing disorders
  • Develop and implement individualized treatment plans
  • Collaborate with an interdisciplinary team including nursing, therapy, and physicians
  • Document evaluations, treatments, and patient progress accurately
  • Educate residents and families on therapy goals and care plans
