A Calling with Impact At Lifted Hospice, our Clinical Liaisons meet patients and families at the bedside, often in moments of crisis—and guide them toward comfort, clarity, and support. You help people understand what hospice is, when it’s needed, and how it brings relief and dignity. You bring clinical expertise into emotionally complex moments, helping physicians trust the timing and families feel safe moving forward. Your presence reassures, informs, and opens the door to care that honors life and brings peace. Your Role in the Mission As a Clinical Liaison, you are the clinical bridge between acute care and hospice care. You serve as the trusted clinical voice of Lifted in hospitals, facilities, and provider offices—translating medical needs into meaningful action. You assess eligibility with discernment, advocate for timely referrals, and equip families to make informed decisions. You work closely with physicians, case managers, and care teams to ensure patients receive the right care at the right time. You embody the Lifted Promise by putting people first, communicating with clarity and kindness, and helping ensure that no one misses the opportunity for care that brings dignity, relief, and peace.

Requirements

  • Graduate of an accredited nursing program (Diploma, Associate, or Bachelor’s) with an active RN or LVN license in good standing.
  • Clinical experience in a healthcare setting is required; hospice, palliative, or hospital-based experience is preferred.
  • Demonstrated clinical competency aligned with current nursing standards of practice.
  • Excellent interpersonal and relationship-building skills, with the ability to engage compassionately and confidently with patients, families, physicians, and referral sources.
  • Strong verbal and written communication skills, including the ability to present hospice services and eligibility clearly to both clinical and non-clinical audiences.
  • Working knowledge of hospice care, referral pathways, and relevant healthcare regulations and standards.
  • Proficient in clinical documentation and EMR/charting; experience with hospice systems is a plus.
  • Strong planning, time management, and independent work skills in a fast-paced, field-based environment.
  • Professional, composed, and adaptable in sensitive or high-pressure situations.
  • Must be able to read, write, and communicate clearly in English.
  • Current driver’s license, reliable transportation, and valid auto liability insurance.

Nice To Haves

  • Specialized knowledge of end-of-life care, symptom management, or clinical decision-making preferred.
  • Experience with hospice systems is a plus.

Responsibilities

  • Assess hospice eligibility – Conduct timely, compassionate clinical evaluations to determine appropriateness for hospice services in alignment with Medicare Conditions of Participation and Lifted standards.
  • Perform and support admissions – Complete on-site assessments and admissions when needed, and facilitate the transition process by gathering documentation and coordinating with the hospice team.
  • Guide patients and families – Provide clear, empathetic education about hospice philosophy, services, and benefits to support informed decision-making.
  • Build referral relationships – Establish and maintain strong connections with hospitals, specialists, long-term care facilities, and healthcare providers to support appropriate and timely referrals.
  • Support outreach strategy – Implement clinical outreach and marketing plans under the direction of the Chief Sales Officer to promote Lifted’s mission and expand access to care.
  • Educate and train others – Deliver in-service education to referral partners and assist in training Hospice Liaisons using specialty clinical roadmaps and field tools.
  • Expand access to higher levels of care – Assist in building and clinically maintaining General Inpatient (GIP) contracts within assigned regions.
  • Represent Lifted with professionalism – Serve as the clinical voice of Lifted Hospice in hospitals, facilities, and provider offices, offering trusted insight and compassionate presence.
  • Collaborate across teams – Work closely with admissions, nursing, outreach, and leadership to ensure a seamless, people-first experience.
  • Maintain accurate documentation – Complete clinical documentation thoroughly and promptly, in compliance with regulatory and Lifted standards.
  • Ensure compliance and integrity – Conduct all outreach, education, and clinical activities in accordance with healthcare laws, ethical standards, and Lifted policies.
  • Remain flexible and responsive – Maintain a responsive schedule to meet the needs of patients and referral partners, including early mornings, evenings, weekends, and local travel.
