About The Position

SUMMARY: The Clinical Manager of Pharmacy and Primary Care is responsible for managing clinical, operational, and staffing aspects of the Clinical Pharmacy and Primary Care teams. In addition, the Clinical Manager of Pharmacy and Primary Care supports optimal clinical, quality and financial outcomes through strategic alignment with Physicians-in-Charge, providing guidance and education to advanced practice providers, fostering high quality medication management and health plan financial stewardship in a collaborative environment. Uses clinical expertise and high-level administrative skills to ensure all team members can practice at the top of their license, resulting in high quality service delivery to participants and families of the PACE program. Works as a member of the PACE Leadership Team to enforce and sustain regulatory compliance, promote cost-improvement strategies, and facilitate and improve appropriate clinical and medication management services. Promotes and provides skill development of pharmacy and advanced practice provider staff in alignment with organizational mission, values and goals. SPECIFIC DUTIES AND FUNCTIONS: Supervises and coordinates scheduling for clinical pharmacists, pharmacy administrative staff, and contingent advanced practice providers (APPs). Evaluates employee work performance. Counsels, disciplines and recommends the termination of subordinates as indicated. Design, implement and assess clinical programs and initiatives, including but not limited to antimicrobial stewardship, complex medication management, palliative medicine, and polypharmacy while maximizing health plan stewardship and patient safety. Monitor clinical quality and patient outcomes by developing and implementing quality protocols, conducting chart audits, and analyzing key performance metrics related to safety, productivity, and participant satisfaction. Partners with the Quality Department and Physicians-in-Charge to plan, educate, develop, and implement goals in order to ensure compliance with medication- and care-related regulations. Develops, implements and maintains quality improvement projects resulting from data analysis. Facilitates medication utilization review, including but not limited to policies and procedures related to high-cost medications, medication management projects, and operationalizing a culture of effective medication cost management. Develops and supports cost-improvement strategies, including the communication of financial metrics and responding to budget variances. Develops, implements and manages evidence-based clinical services that optimize participant outcomes across all centers in a collaborative environment with advanced practice providers, including facilitation, course correction, and analysis of medical utilization trends. Develops departmental budgets and manages financial performance metrics that track ways to optimize medication use, reduce costs, and support value-based care models. Oversees training of advanced clinical personnel to ensure staff competency and synchronization of clinical services across all centers. Supports clinical advancement of APPs and clinical pharmacists through professional presentation and educational opportunities. Serves as a bridge between care teams working closely with physicians, APPs, pharmacists, clinical leadership, center leadership, and other departments to ensure seamless and integrated patient care. Develops departmental policies and procedures to ensure safe, appropriate and cost-effective use of medications in collaboration with the PACE IDT. Ensures compliance with contractual, regulatory, and PACE departmental policies and procedures. Ensures the achievement of operational goals and standards by communicating, directing, and supporting program and departmental operations. Performs quality initiative, clinical improvement or other project management projects as assigned.

Requirements

  • An advanced clinical degree, such as Doctor of Pharmacy (PharmD), Master’s degree in Nursing (for a Certified Registered Nurse Practitioner), or Physician’s Assistant.
  • An active, relevant state license and potentially a board certification in a specialty area, such as ambulatory care or geriatric practice.
  • At least five years of clinical patient-care experience including one year with a frail elderly population, advanced medication management knowledge, and at least three years of clinical leadership or supervisory experience.
  • Ability to lead and direct other licensed mid-level clinicians, such as pharmacists and APPs in the delivery of care and achievement of goals within the scope of their practice.
  • Minimum three years of clinical experience in a patient-facing role.
  • Strong leadership, communication and organizational skills in addition to experience in quality improvement, program development, and data analysis.
  • Group leadership skills.
  • High level of professional and analytical skills to solve problems and implement solutions.
  • Ability to critically evaluate clinical pharmacy and primary care services and implement quality improvements.
  • Ability to establish and maintain interpersonal and interdepartmental relationships.
  • Ability to lead and direct interdisciplinary licensed clinical professionals.
