We specialize in home health care services that enable the patient to adapt and manage his or her medical situation in the comfort of their home. Stella Maris offers home health services to residents of Baltimore City, Baltimore County, Carroll County, Harford County and Howard County. We are currently looking for a Clinical Manager (RN) to provide leadership for the growing Home Care team. This is an exciting opportunity to lead, develop, supervise, and mentor a team of field nurses and clinicians to meet the needs of clients in the community. To develop, coordinate, direct, and evaluate all clinical services to the Home Health program. The ideal candidate will have extensive Home Care and leadership experience. This is an integral role within Stella Maris.

Requirements

  • Associates Degree (BSN preferred)
  • RN or PT license in Maryland
  • 5 years experience in home health leadership role
  • Strong communication, computer, community relations, and personnel skills

Responsibilities

  • Works closely with the administrator to assist in development of a business plan, strategic goals, resource allocation plan, and a yearly budget for the Home Health Department.
  • Consults with the administrator in policy formulation and implementation ensuring consistency with standards of practice, industry best practices and State and Federal regulatory agencies
  • Develop clinical care pathways using evidence based practice to achieve optimal clinical outcomes ensuring patient safety and improving efficiency.
  • Participates in QAPI plans & processes and oversees all Q/A programs and utilization review. Responsible for measuring overall quality of care and developing initiatives for improvement in quality measures relative to STARS, HHCAHPS, HHC, HHVBP and other industry standards.
  • Evaluates the program, services, and field personnel. Monitors and evaluates employee performance, holding employees responsible for maintaining productivity and achieving established goals and objectives. Provides timely performance evaluation of personnel.
  • Acts as liaison with Staff Development for program areas. Works closely with the department Educator to ensure smooth onboarding and appropriate orientation of new personnel and continued development of all staff through coaching, mentoring and regularly scheduled training and education
  • Assures appropriate staffing levels, on-call and supervision during all service hours.
  • Oversees all patient care services and staff including intake and scheduling of new and existing patients assuring that patient needs are assessed and that an appropriate individualized plan of care is developed, implemented and updated.
