About The Position

The Nursing Supervisor is responsible for direct oversight and supervision of a team of Nurses and Medical Assistants assigned at their site(s). Provides a leadership role to nursing and medical assistant staff including recruitment, personnel management, day to day supervision, guidance and training/mentoring. Promotes the delivery of coordinated, high quality patient care to a diverse population and provides direct clinical nursing care at designated site. Is a contributing member of the triad site leadership team and promotes collaboration with medical and operations staff. Promotes high quality patient care by leading safety and quality initiatives at the site level. Works as a liaison between nursing leadership, nursing staff, patients and other CHCB staff and outside organizations as necessary.

Requirements

  • Bachelor of Science in Nursing
  • Good standing with the Vermont State Board of Nursing
  • Three years of full-time experience as an RN strongly preferred
  • One (1) year of nursing leadership, supervision or clinical management experience required
  • Ability to manage personnel issues including performance, hiring, evaluations and disciplinary actions.
  • Knowledge and proficiency in nursing standards of practice, including assessment, evaluations and plan, documentation, etc.
  • Knowledge of human behavior, differences in ability, personality and interests; learning and motivation.
  • Ability to motivate, develop and direct staff to promote a strong cohesive team.
  • Ability to identify areas for improvement/enhancement to patient care and make appropriate recommendations.
  • Knowledge of business and management principles involved in planning and resource allocation.
  • Excellent verbal and written communication skills.
  • Proficiency in computer technology applicable to clinical operations, such as Excel, PowerPoint and Word.
  • Excellent attention to detail and ability to multi-task to meet performance expectations.
  • Ability to maintain professional poise and calm demeanor in stressful situations.
  • Ability to adhere to work schedule in a punctual and reliable manner and willingness to work flexible hours.
  • Ability to work independently, to solve problems creatively and with initiative to improve care to our patient population.
  • Vermont State Registered Nurse Licensure
  • Basic Life Support (BLS) Certification

Responsibilities

  • Responsible for the oversight and day to day management of the nursing team and clinical operations.
  • Maintains adequate Nursing and Medical Assistant staffing/scheduling to meet daily clinic needs.
  • Supervises and evaluates daily work activities of nursing personnel to ensure appropriate distribution of workload.
  • Assures efficient patient flow for patient appointments.
  • Provides staff with guidance in handling difficult or complex problems and in resolving complaints or disputes. Appropriately escalates disputes to the attention of the Nurse Manager for further resolution as needed.
  • Collaborates with Clinical Educators to assist with the training of new staff and promotes and provides an environment of continuous learning for existing staff.
  • Participates in the budget development and monitoring for area of responsibility with specific attention to clinical personnel and equipment.
  • Serves as site leader for nursing and organizational clinical improvement projects and initiatives.
  • Identifies opportunities to improve patient care, patient safety and quality and leads efforts to develop process improvements.
  • Ensures continuity of care, meeting quality metrics and enhancing reimbursement when appropriate.
  • Partner with Practice Manager/Supervisor and Associate Medical Director as member of site-based leadershp team.
  • Oversees inventory management and ordering of clinical supplies within budgetary limitations.
  • Ensure adequate supply and proper use of medical equipment.
  • Encourages and models open communication between staff and other departments, ensuring effective interdepartmental functioning. Makes recommendations as appropriate.
  • Identifies and recommends opportunities for improvement: designs and implements process improvement activities.
  • Ensures departmental work activities are in compliance with CHCB policies, procedures and protocols.
  • Ensures compliance with applicable governing laws/regulations and guidelines such as HIPAA, OSHA, Vermont Department of Health, FQHC’s, Vermont State Board of Nursing, health insurance regulations, etc.
  • Ensures removal of Biohazard materials according to protocol.
  • Responsible for all personnel management activities including recruiting, hiring, performance reviews, performance improvement.
  • Supports professional development of staff, including identification of educational opportunities, mentoring and goal development.
  • Conducts regular staff meetings and communicates nursing, site and organizational priorities.
  • Meets with staff individually to provide mentoring, coaching and professional development.
  • Provides direct nursing care to patients.
  • Consults and coordinates with health care team members to assess, plan, implement and evaluate patient care plans.
  • Performs standard and specialized diagnostic tests and procedures independently according to nursing protocols or under the direction of a provider and in conjunction with the lab (including but not limited to venipuncture, immunizations, EKGs, lab testing, medication administration, nebulizer treatments and spirometry).
  • Evaluates medication requests and facilitates prescription refills following the CHCB medication refill policy; dispenses stock medications under the direction of the provider and according to CHCB policy.
  • Provide comprehensive triage assessment and develops plan of care for patients via phone and walk-in and documents per nursing standards.
  • Modifies patient treatment plans as indicated by patients' responses and conditions; monitors all aspects of patient care.
  • Demonstrates competency in Laboratory procedures and provides basic lab coverage as defined as agreed to between nursing and lab leadership.
  • Records all patient interactions appropriately in the EHR.
  • Provides on-going patient and family education, assesses patient knowledge and understanding of disease process and treatments, facilitates follow up care and reviews provider instructions to ensure understanding.
  • Instructs patients, families and other groups on topics such as health education, disease prevention and childbirth; provides health education materials. Educates patients with respect to educational and cultural background.
  • Facilitates patient care activities and coordinates flow of patients in the clinical setting.
