About The Position

Assesses participant’s physical and functional needs, along with psychosocial status and needs, utilizing professional knowledge, skills of observations and interviewing skills. Documents change in the participant’s condition noted in clinic and inform appropriate care members. Regularly informs the interdisciplinary team of the medical, functional, and psychosocial condition of each participant. Assumes responsibility for continued professional growth, such as in-service programs. Responsible for day to day operation and activities of the clinic Responsible to ensure that clinical procedures requested are performed in the appropriate time frame Interface with transportation companies to ensure residents will be transported to appointments. Deal effectively, professionally and tactfully with residents and families. ( add participant instead of resident Perform TB test or QuantiFERON blood test for employees when indicated Administer flu and COVID to employees when needed. Administer influenza, pneumonia, hepatitis and PPD to participants Demonstrates the ability to recognize priorities in organization of workflow. Able to perform duties independently with minimal need for direct supervision. Responsible for tracking and documenting PHS for participants Responsible for daily and weekly logs in clinic. Responsible for ensuring any item that is expiring is reordered Notify nurse manager or Program Director when equipment not working properly. Ensure steady follow of clinic Coordinate with scheduler when a ppt is due to be seen in clinic. Perform other duties as assigned.

Requirements

  • Current NY State Registered Nurse License
  • Associate Degree
