Reporting directly to the Clinical Operations Resource Nurse Supervisor, the Clinical Operations Resource Nurse is responsible for providing support to home care and hospice patients through telephone triage. He/she are responsible for prioritizing patient care needs by assessing, managing and evaluating new or worsening symptoms. This role also collaborates with and supports other teams whose work requires information obtained through the triage process. The Clinical Resource Nurse works collaboratively with home care and hospice leadership and clinical teams.

Requirements

  • Graduate of an accredited school of Nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC)
  • Registered Nurse with one to two years of home health or hospice experience
  • One to two years of telephone triage experience

Responsibilities

  • Clinically triages phone calls from patients/families/caregiver by providing skilled and palliative care assessment, symptom management, problem solving and instruction.
  • Assists with the coordination of home visits as appropriate for patients who call the Agency seeking guidance.
  • Clinically coordinates the scheduling of complex IV, Medication Reconciliation and Wounds Care referrals.
  • Provides guidance including pain management and symptom control; conducts assessments and evaluations and provides education and support care to patients and caregivers in collaboration with the nurse case manager.
  • Coordination of patient care with the physician office and enters orders as indicated. Communicates outcome with clinical team.
  • Receives and contacts by phone/fax/email any patient physician (designee) as indicated by the change in the patient’s condition, treatment and/or status.
  • Accepts and completes all necessary documentation related to accepting medications and/or Durable Medical Equipment for Hospice patients.
  • Completes comprehensive documentation about the calls received/initiated in the patient’s medical record using the SBAR guide.
  • Assists daily with clinical triage of community and facility nursing and therapy visits and admissions for both home care and hospice programs.
  • Supports scheduling with determining priority of assigning open visits.
  • Assists with chart review and monitoring of Quality Improvement and Patient Safety activities as requested.
  • Reviews orders that impact the patient’s plan of care and makes appropriate changes.
  • Acts as a resource to the Home Care and Hospice Teams.
  • Demonstrates flexibility in the role by accepting other duties as assigned for which qualified
