About The Position

We are seeking an experienced Clinical Pediatric Intake Supervisor (RN) to lead the intake and admission process for pediatric patients referred to our home health agency. This role is responsible for supervising pediatric intake staff, evaluating referrals, coordinating with physicians and families, and ensuring timely admissions in accordance with regulatory standards and pediatric best practices. The ideal candidate has strong pediatric clinical experience, leadership ability, and knowledge of home health services. Key Responsibilities Supervise and coordinate pediatric referral intake, screening, and admissions Evaluate pediatric referrals to determine eligibility for home health services Provide clinical oversight and guidance to intake staff Coordinate communication with physicians, hospitals, case managers, and families Ensure Start of Care documentation and EMR records are completed accurately Establish and confirm physician orders and pediatric plans of care Assist with complex or high-acuity pediatric referrals Monitor intake workflow to ensure timely admissions Ensure compliance with federal, state, and agency home health regulations Support quality improvement initiatives and intake process optimization

Requirements

  • Active RN license in New York State
  • BSN required
  • 2+ years pediatric nursing experience (acute care, home health, or case management)
  • Pediatric clinical expertise
  • Strong leadership and communication skills
  • Knowledge of Medicare, Medicaid, and managed care regulations
  • Ability to manage multiple referrals and prioritize workflow
  • Experience collaborating with interdisciplinary teams

  • Prior leadership or supervisory experience
  • Experience in home health intake or care coordination
