Responsible for creating, directing, coordinating and evaluating a Performance Improvement plan that meets, at a minimum, the requirements under Title 42 CFR § 460, Subpart H – Quality Assessment and Performance Improvement (QAPI). Quality monitoring and performance improvements must be established for the multiple regulated and un-regulated medical, institutional and community-based settings necessary to providing quality, person-centered and cost-effective care consistent with the National Program of All-inclusive Care for the Elderly (PACE) model. Care settings include the adult day care center, Keyser Clinic, contracted assisted living facilities, skilled and unskilled in-home care, contracted acute facilities, skilled and long-term nursing facilities, and transportation services. This position is responsible for establishing and monitoring quality and performance metrics that support the clinical and insurance functions of a PACE program as they affect participant safety and satisfaction. Responsible for ensuring compliance with federal and state regulatory quality reporting. Reports to the Director, Hopkins ElderPlus and takes direction from the Medical Director in the development of quality goals. Member of the Hopkins ElderPlus Management Team.
