The Clinical Pharmacist – Inpatient role is based at Cleveland Clinic South Pointe Hospital where the caregiver works as a decentralized pharmacist primarily supporting the Internal Medicine service line. In this role, the pharmacist collaborates closely with providers, nurses, pharmacists, pharmacy technicians, leadership, and other members of the patient care team to optimize medication therapy and support safe, effective patient care. Responsibilities include participating in multidisciplinary rounds, providing medication management support for hospitalized patients, and contributing to clinical decision-making within the Internal Medicine units. The caregiver may also assist with centralized pharmacy operations as needed, ensuring continuity of pharmacy services and medication distribution across the hospital. This is a PRN role. A caregiver in this position works varying shifts predominately during the day. Requirements include 1 holiday per year and every 5th weekend.

Requirements

  • PharmD or Bachelor’s of Science in Pharmacy from an accredited college of Pharmacy
  • Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire
  • Current Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross and completion of a Board of Pharmacy approved course in the administration of immunizations or certification within 90 days of hire for individuals administering immunizations

Nice To Haves

  • Board certification
  • Completion of a post-graduate year 1 residency (PGY1), fellowship or relevant post-graduate pharmacy experience preferred
  • Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area (i.e. Critical Care, Oncology)

Responsibilities

  • Provide safe, efficient, patient-centered and evidence-based medication services.
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.
  • Document services in a complete, accurate and timely manner.
  • Ensure all pharmacy operations comply with national accreditation standards.
  • Maintain professional interactions with patients to facilitate successful patient experiences.
  • Educate and train students, residents, other caregivers and/or patients.
