About The Position

This role is responsible for delivering comprehensive clinical pharmacy services, ensuring safe and effective medication use, and supporting a diverse patient population, including pediatric and critical care patients. Requirements

• Minimum 1 year of recent Clinical Pharmacist experience in an acute care hospital setting (required)
• Bachelor’s or PharmD from an accredited School of Pharmacy
• Active Pharmacist license in good standing (state-specific requirement)
• Strong knowledge of pharmacokinetics, IV to PO conversions, and clinical documentation

Responsibilities

• Provide clinical consultations including pharmacokinetics, nutrition support, anticoagulation monitoring, and renal dosing
• Review medication regimens for appropriateness, interactions, and allergies
• Support formulary management and therapeutic interchange initiatives
• Accurately verify, compound, and dispense medications
• Supervise pharmacy technicians and oversee daily operational workflow
• Document clinical interventions, medication errors, and adverse drug reactions
• Deliver medication education to healthcare providers and patients
