About The Position

Clearway Health is seeking a Clinical Pharmacist! This position ensures optimal patient outcomes related to chronic and specialty drug-related disease states. Your tasks include preventing or resolving medication-related issues and optimizing drug therapy. You serve as a resource and drug expert to patients, families, physicians, nurses, and other healthcare professionals. Additionally, plays an integral role in educating pharmacists, residents, and students. As needed, may also be involved in coordinating the acquisition and distribution of drug therapy. This position will be located on site at our client partner, Signature Healthcare, founded as Brockton Hospital in 1896, a not-for-profit, integrated healthcare system providing a wide range of services to patients of all ages throughout Southeastern Massachusetts. They are comprised of the award-winning Signature Healthcare Brockton Hospital, Signature Medical Group (SMG), a multi-specialty physician group of more than 150 physicians practicing in 15 ambulatory locations, and the Brockton Hospital School of Nursing. Requirements

  • A PharmD or bachelor's degree from an accredited College of Pharmacy is required.
  • Completion of PGY-1 Pharmacy Residency or 3+ years working as a clinical pharmacist in direct patient care is required.
  • An active Pharmacist licensure (in good standing) to practice pharmacy in the state in which you are applying in or and/or the ability to acquire this at the time of hire is required.
  • An active Pharmacist vaccination certification in the state of Massachusetts
  • Must possess or be able to obtain a State Board of Pharmacy Preceptor’s license by the current state you are applying to within one year of hire.
  • 2+ years of work experience in ambulatory care pharmacy setting post PGY1 training or 3+ years post PharmD is preferred.
  • Demonstrated expertise in medications, drug interactions, medical procedures, therapies, diagnoses, and disease states is essential.
  • Exceptional interpersonal skills for effective communication with diverse stakeholders including hospital staff, patients, families, physicians, and colleagues.
  • Ability to effectively manage multiple tasks, prioritize duties, and adhere to deadlines.
  • Organized work approach with a proficiency in maintaining accurate documentation.
  • Strong verbal and written communication skills; proficiency in additional languages is advantageous.
  • Self-directed with strong self-management abilities; capable of identifying and addressing issues independently.
  • Proven track record in process improvement and a keen eye for detail and precision.
  • Proficiency in Microsoft Office programs (e.g., Word, Excel, Outlook) and web browsers.

Nice To Haves

  • Completion of a PGY-2 Ambulatory Care or Specialty Pharmacy Residency/Fellowship) is preferred.

Responsibilities

  • Provide direct patient care in an ambulatory setting, focusing on managing chronic and specialty drug-related disease states.
  • Conduct health and medication histories and perform limited physical examinations as necessary to evaluate medication therapy effectiveness.
  • Evaluate patient response to medication therapy through direct involvement and clinical assessment of findings.
  • Collaborate with referring physicians to optimize pharmacotherapy according to standard care guidelines for optimal patient outcomes.
  • Monitor medication therapy, making modifications based on laboratory values and clinical response.
  • Educate patients and caregivers on medication and disease states.
  • Assist in managing medication-related problems in clinics and act as the drug information resource.
  • Serve as a liaison between outpatient pharmacies and clinic staff, aiding in resolving third-party coverage barriers.
  • Document patient interactions in the electronic medical record.
  • Identify and implement cost-saving and containment programs.
  • Participate in adverse drug reaction and medication error reporting programs.
  • Implement quality improvement initiatives in the ambulatory setting.
  • Precept pharmacy residents and students and provide in-service education.
  • Participate in medication-related research and present results at meetings.
  • Represent the department in hospital committees and attend staff development activities.
  • Maintain professional competency through continued education.
  • Develop fiscal year goals aligned with institutional objectives.
  • Support 340b program operations, including patient identification and staff education.
  • Assist with prescription filling, typing, or delivery on a limited basis.
  • Create and maintain a healthy work environment by fostering communication, accountability, and ethical conduct.
  • Engage in departmental initiatives and uphold organizational values.
  • Adhere to hospital infection control and safety procedures
