This position integrates both clinical responsibilities as a Clinical Pharmacy Specialist and administrative duties as a Quality Coordinator. Under direction of the Clinical Pharmacy Director, the Clinical Pharmacy Specialist and Quality Coordinator will assist in the planning, organizing, and monitoring of the delivery of ambulatory care pharmacy services for the Chase Brexton Health Care Centers. They will provide oversight of the planning, implementation, and evaluation of the DSMES service across all sites, ensuring consistent quality and compliance to national standards. In this role, the Clinical Pharmacy Specialist and Quality Coordinator provides pharmaceutical care services through direct interaction with patients and providers, at the highest level of clinical practice, working in close collaboration with other clinical team members and serve as a consultant for intensive medication therapy management services. They may also serve as pharmacy representatives to health center committees as requested; assist in student preceptor programs; participate in research programs where appropriate; and provide appropriate leadership to other pharmacy staff. Additionally, they oversee daily Diabetes Self-Management Education and Support (DSMES) operations across all sites and ensure continuous compliance with the National Standards for DSMES. They will play a key role in future clinical initiatives by contributing to the development, implementation, and ongoing monitoring of new programs and quality‑improvement efforts.
