Hospice Clinical Team Lead (RN) Shift Available: Full Time Service Area for Coverage: Greater Houston
Requirements
- Valid RN license in the state of Texas
- Hospice leadership experience (at least 1 year)
- Excellent documentation and clinical assessment skills
- Knowledge of home health/hospice regulations and general healthcare compliance
- Excellent communication and people skills are essential
- Ability to work independently and as part of a team
Nice To Haves
- Experience with Home Care Home Base (HCHB) EMR (preferred but not required)
Responsibilities
- Oversee the multidisciplinary team providing hospice care
- Provide clinical leadership while ensuring the delivery of high quality end-of-life care
- Review workflow, and monitor productivity
- Review and resolve documentation issues
- Ensure compliance with federal and state regulatory requirements
- Facilitate communication between families, patients, and healthcare providers
