Located on the Community Regional campus, Burnett Extended Care Center is our brand-new 154-bed transitional care facility opening this summer. Purpose-built to serve patients transitioning from hospital to home, Burnett includes specialized units — Memory Care, Long-Term Care and Transitional Care — that focus on quality, comfort and continuity of care. This environment offers nurses the opportunity to provide patient-centered care across diverse needs in a collaborative, post-acute setting. Assist residents with activities of daily living such as bathing, dressing, grooming, toileting, and mobility support. Support RNs and LVNs by observing residents, documenting care, and promptly reporting any changes in condition. Deliver direct, hands-on care to residents requiring 24-hour medical supervision, ensuring safety and comfort throughout their recovery or long-term stay. Requirements

High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
Certified Nursing Assistant training to obtain certification required
CNA - State of California Certified Nursing Assistant Certification required
BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required

Responsibilities

Assist residents with activities of daily living such as bathing, dressing, grooming, toileting, and mobility support.
Support RNs and LVNs by observing residents, documenting care, and promptly reporting any changes in condition.
