About The Position

About the Role Always Best Care is hiring a Client & Community Care Manager to help grow our local O‘ahu business through high-quality initial assessments and strong community/referral relationships . This is a non-clinical role that combines intake/coordination with community outreach. We hold both Home Care and Home Health licenses , and this position plays a key role in ensuring clients are connected to the right level of care — including referring appropriate clients/patients to our Home Health entity when skilled services may be needed (in coordination with our internal team). What You’ll Do Client Intake & Assessments Conduct initial non-clinical assessments in homes and facilities (hospital, SNF, assisted living, etc.) Educate clients/families on services and next steps Coordinate a smooth hand-off to operations once care starts Provide light follow-ups (30/60/90 days and issues as needed) Care Continuum Coordination (Home Care ↔ Home Health) Identify when a client may benefit from skilled home health services Refer/route appropriate clients to our Home Health (dba) team for next steps Support continuity and communication across services (no clinical decision-making) Community & Referral Relationship Building Build relationships with hospitals, case managers/social workers, facilities, physicians’ offices, and community partners Represent Always Best Care at meetings/events and maintain a strong local presence Support growth through relationship-based marketing (no hard quotas) What Success Looks Like More referrals and stronger community visibility Great first impressions and smooth starts of care Clients matched to the right service, including Home Health when appropriate Trusted relationships across O‘ahu’s healthcare and senior services network

Requirements

  • Non-clinical (no license required)
  • Strong people skills, organization, and follow-through
  • Comfortable working independently in the field and coordinating with a team
  • Reliable vehicle + ability to drive across O‘ahu frequently
  • Schedule flexibility depending on caseload (occasional evenings/weekends)

Nice To Haves

  • Local knowledge of O‘ahu healthcare/senior services is strongly preferred
