About The Position

LifeLong Medical Care is looking for a Community Engagement Specialist to work at our 7th Street site in Berkeley. In collaboration with the City of Berkeley’s Public Health Division, the Community Engagement Specialist is responsible for planning, implementation, coordination and evaluation of the Heart 2 Heart Neighborhood Health Advocate Program, a program dedicated to capacity building efforts of vulnerable and underserved populations in South Berkeley. Working in collaboration with the members of the health outreach team, the Community Engagement Specialist will also be responsible for staffing and implementation of hypertension drop-in clinics at various locations, as well as other community activities that support LifeLong’s broader community outreach efforts. This is a full time, benefit eligible, in-person position. Bilingual English/Spanish highly preferred. LifeLong Medical Care is a multi-site, Federally Qualified Health Center (FQHC) with a rich history of providing innovative healthcare and social services to a wonderfully diverse patient community. Our patient-centered health home is a dynamic place to work, practice, and grow. We have over 15 primary care health centers and deliver integrated services including psychosocial, referrals, chronic disease management, dental, health education, home visits, and much, much more.

Requirements

  • Familiarity with South/West Berkeley, and/or a passion for this community
  • Clear understanding of the health issues (hypertension/cardio-vascular disease, etc.) that disproportionately affect the South/West Berkeley community
  • Experience with and/or knowledge of health inequities and the impacts of Social Determinants of Health
  • Excellent interpersonal, verbal, and written skills; strong problem-solving and organization skills; ability to multi-task
  • Ability to work with diverse communities of people with different backgrounds in a culturally appropriate manner
  • Commitment to the provision of primary care services for the underserved and low-income populations
  • Work effectively under pressure and in a team-based environment; complete assignments in a positive, professional and timely manner and ability to be flexible and adaptive to change
  • Effectively present information to others, including other employees, community partners, and vendors
  • Conduct oneself in internal and external settings in a way that reflects positively on LifeLong Medical Care
  • Skills and experience with Microsoft Office programs – Outlook, Excel and Word
  • One to two years’ experience working with a non-profit organization and/or experience in community engagement or health outreach
  • Must be able to work some evenings and weekends
  • Must have a reliable source of transportation to travel within Contra Costa County and Alameda County

Nice To Haves

  • Bachelor’s degree or vocational degree in related field
  • Bilingual in Spanish preferred
  • Experience with diverse employee populations and multi-location organizations a plus

Responsibilities

  • Responsible for implementing, coordinating and evaluating Heart 2 Heart’s Neighborhood Health Advocate Program and activities
  • Identify, initiate, and deepen relationships with various internal and external stakeholders to reduce cardiovascular health inequities in the Heart 2 Heart community
  • Empower and support individuals that live, work, play, and/or pray within this neighborhood in their efforts in strengthening a healthy and vibrant community
  • Develop and deliver cardiovascular health education in collaboration with community partners towards lowering cardiovascular disease and enhancing community and individual health in the Heart 2 Heart neighborhood
  • Recruit and supervise Health Advocates and youth interns recruited from underserved neighborhoods in South Berkeley to develop health advocacy training expertise and foster leadership and professional development
  • Staff hypertension drop-in clinics at various locations, including data collection, data entry, and generation of reports
  • Facilitate on-going trainings and meetings
  • Research and identify appropriate health advocacy curriculum material for targeted underserved populations
  • Prepare timely and accurate updates and reports of community health advocacy activities to the Community Engagement Manager
  • Utilizes evaluation tools to measure program outcomes and effectiveness
  • Performs other duties and projects as needed or assigned by the Community Engagement Manager
