Certified Community Health Worker Certification is highly preferred Position Purpose: This role is responsible for working with individuals or groups that are enrolled in Medicaid or Medicare to deliver a range of activities such as outreach, community education, informal guidance and support. Works with community organizations who support individuals with health and life needs/goals such as food, housing, transportation, etc. Conducts telephonic and in person outreach to locate individuals and families. Provides education or support for health, life needs and/or social services. Conducts basic non-clinical guideline assessments and refer to appropriate services, resolve concerns on member’s behalf based on guidelines, and gather information for medical providers and staff working within our organization. Conduct other guidelines assessments such as home safety, assessment of the community/environment and resources, and assessment of non-medical needs such as transportation, employment, etc. and refer members to appropriate services, resolve concerns on members behalf based on guidelines. Works with community organizations that support individuals with health and life needs/goals such as food, housing, transportation, etc. Participate in coordination, follow up, and resolution of medical and non-medical needs including appointment tracking, documentation, referrals and follow up, facilitating transportation to services, etc. Provide information to staff on vulnerable communities, including awareness and education on vulnerable communities. Must make frequent visits to individual homes and community organizations.

  • High School Diploma or GED.

Nice To Haves

  • Certified Community Health Worker Certification is highly preferred
  • Current or previous enrollment in Medicaid or Medicare preferred.
  • Bilingual skills preferred.
