About The Position

Works under the general supervision of the CHOICES Manager to receive referrals from Managed Care Organizations (MCO), in a method prescribed by TennCare, for each member choosing to receive Community Living Supports (CLS) and determined by the MCO to be appropriate for the CLS benefit. Schedules and conducts visits with referred members at their residence within the four (4) county areas of Shelby, Fayette, Lauderdale, and Tipton counties. Educates members on services, settings, CLS providers, housing options, and available community support. Also communicates with the MCOs about transition concerns and reports all abuse and/or neglect concerns to the proper authorities.

Requirements

  • Three (3) years of work experience in case management, social work, education, or advocacy serving older adults and/or adults with physical disabilities in long term care programs; or conflict resolution experience, with at least two (2) years, in aging, long-term care, or related field; AND
  • Bachelor’s degree from an accredited college or university in Social Science, Human Services/Human Relations, Social Welfare, or related field;
  • An equivalent combination of related education and/or experience.
  • PROOF OF EDUCATION, TRAINING, AND/OR EXPERIENCE IS REQUIRED.
  • Must possess a valid Tennessee driver's license or secure one upon date of employment.
  • Must have daily access to an operable automobile and meet County standards for automobile insurance upon date of employment.
  • Must successfully pass a background check to include: Tennessee Abuse Registry, Tennessee Felony Offender Registry, National and Tennessee Sexual Offender Registry.

Responsibilities

  • Receives referrals from MCOs and schedules and conducts an initial visit within ten (10) days of receipt of referral.
  • Conducts face-to-face pre-transition screening visit at member's current residence.
  • Provides general CLS education, including individual choice of services, settings, providers, and housemates.
  • Addresses quality and other concerns, such as the role of the providers, Care Coordinator, and MCO Member Advocate.
  • Documents information and member visits in Mon Ami software.
  • Maintains and updates files on each member referred.
  • Assists in resolving complaints and preparing required reports as relates to CLS.
  • Facilitates referrals to Adult Protective Service for abuse or neglect situations.
  • Provides recommendations to TennCare concerning improvements to CLS benefits.
  • Attend trainings related to the CHOICES Ombudsman and Home and Community-Based Services (HCBS).
  • Completes Choices in home assessments as needed when Intake Assessors caseload is over the contracted amount.
  • Performs other related duties as required or directed.
