Position Overview: The PACT Licensed Practical Nurse (LPN) supports the delivery of medication monitoring, administration of prescribed IM medication, and taking and tracking of vital signs in coordination with the team RN. The LPN is responsible for working to full scope of practice, performing work as required and functioning as an effective member of the interdisciplinary team.

Requirements

  • High school diploma or equivalent, must have graduated from an accredited nursing training program and possess and maintain licensure, in good standing, in the State of NJ as an LPN.
  • Current CPR Certificate required (BLS)

  • Valid driver’s license required
  • No more than one moving violation within the past 12 months
  • Vehicle required

Responsibilities

  • As part of the PACT team, the LPN provides ongoing service coordination and links service recipients with resources.
  • Provide evidence-based nursing care within full scope of LPN practice
  • Obtain and update medical histories and medical information on the EMR, using appropriate evidence based assessment techniques and instruments during a person served visit, with the ability to pass this information onto the PACT Registered Nurse
  • Schedule and coordinate referrals for tests, follow up to other disciplines and utilizing community resources as needed for persons served by the team(s)
  • Able to assist with medication administration as assigned
  • Obtain and document weight, height, girth, and vital signs
  • Assist with implementation of new protocols and act as resource to other staff
  • Maintain therapeutic nurse-person served relationships
  • Provide updates on persons served medical status and needs to mental health treatment team and coordinate care with team
  • Involved in the development, management, and evaluation of the individual, with the family, and other healthcare providers in planning mutually expected outcomes
  • Provide health teaching, addressing topics such as healthy lifestyle, risk reduction behaviors and preventative self-care
  • Participate in quality improvement activities in collaboration with the PACT Registered Nurse
  • Participate in ongoing educational activities related to learning needs and professional issues
  • Serve as a person served advocate and assist him/her to advocate for themselves
  • Administer vaccines and IM injections deemed necessary by provider during the visit, and may draw blood samples or collect urine samples for lab testing as necessary
  • Actively seek out supervision from own supervisor at least monthly, and participate in group nursing practice supervision as assigned
